Abstract

In the article devoted to the 135th anniversary of the G.A. Albrecht Federal Scientific Centre for the Rehabilitation of the Disabled of The Ministry of Labour and Social Protection of the Russian Federation it is said about the main directions of the activity of the Institute of Rehabilitation and Abilitation of the Disabled. The Centre made a significant contribution to the formation and development of the foundations of medical and labour, and later medical and social expertise, social and labour, professional and social rehabilitation. The researches of the physiological functions of the disabled in the process of their work conditions were holding during many years in the Institute. The scientific potential of the centre in modern conditions consists of the development of two institutions that are part of the centre: the Institute of Rehabilitation and Abilitation and the Institute of Prosthetics and Orthotics. The researches currently are related to the implementation of the rights of the disabled and monitoring the implementation of the Convention on the Rights of the Disabled (UN, 2006), the creation of organizational and legal mechanisms for the functioning of the rehabilitation system for disabled in the Russian Federation, the creation of an early assistance system, the development of methodological foundations for the creation of an accessible environment for life of the disabled.

Abstract

According to regulatory documents and published studies, the time in the hospital for rehabilitation after a stroke during one stage is 10-30 hours or more per patient. At the same time, a reliable assessment of the required volume of motor rehabilitation and motor training remains an actual and complex task, which requires special attention of researchers and consensus solutions in order to obtain clear justifications for choosing the type and mode of employment, and to improve the effectiveness of the measures taken.

INFLUENCE OF THE COMPLEX CORRECTION TECHNIQUE TO STRENGTH OF THE QUADRICEPS FEMORIS MUSCLE OF PATIENTS WITH PATELLOFEMORAL PAIN SYNDROME

Abstract

In 40 patients with patellofemoral pain syndrome, before and after the course of treatment, the strength indices of the hip muscles were studied by isokinetic dynamometry, as well as the severity of the pain syndrome in the knee joint and the lumbosacral spine according to the visual analogue scale. The patients of the main group (n = 20) used the technique of complex correction of the pelvic region and lower limb, which was mainly aimed at stabilizing the lumbosacral spine. Patients of the control group (n = 20) performed therapeutic gymnastics aimed at training the muscles of only the interested lower limb. The treatment was performed on an outpatient basis daily for 21 days. After the course of treatment in patients of the main group, the strengths of the quadriceps femoris were significantly higher (p < 0.05), and the degree of pain in the knee joint and lumbar spine was significantly lower (p < 0.05). The obtained data confirm the necessity and expediency of including exercises for stabilization of the lumbosacral spine in the rehabilitation program for patients with patellofemoral pain syndrome.

REHABILITATION MEASURES OF DENTAL DIRECTION IN PATIENTS WITH JUVENILE RHEUMATOID ARTHRITIS IN SANATORIUM-RESORT CONDITIONS

Abstract

Physiotherapeutic procedures normalize physicochemical processes at the cellular and molecular levels. The complex application of natural factors for reducing the interference of the leading pathological focus and deviations of other localization, including the maxillofacial region is actual. The purpose of the study is the development of a therapeutic and prophylactic dental complex for patients with juvenile rheumatoid arthritis (JRA) staying in sanatorium. Material and methods. 30 patients with JRA in the Evpatoriya were examined. Patients of the comparison group receved standart therapy. Patients of the main group additionally received bioresonance stimulation (BRS) of the collar zone and projection of large salivary glands; the oral cavity was rinsed with a solution of the brine from Saky’s Lake. Dental indices, rate of salivation, saliva viscosity, cortisol and ACTH in oral fluid and blood were determined. Results. The effectiveness of dental care in resort conditions was noted in both groups, but the reduction of rate of salivation and viscosity of oral fluid was more dynamic (on 31,58% and 15,6%), reflecting the ability of the BRS to regulate functional activity salivary glands. Cortisol in the OF decreased, which indirectly indicated the physiological activity of the excretory apparatus of the salivary glands at the level of transport molecular metabolism. Conclusions. Staying patients with JRA in the sanatorium-resort conditions in Evpatoriya has a general health effect. “Small” balneotherapy of the brine from Saky’s Lake improves oral hygiene. BRS-therapy restores the functional activity of salivary glands, promotes regulation of cortisol and ACTH levels. Providing rehabilitation dental care for patients with JRA, in the conditions of the resort is justified.

CLINICAL EXPERIENCE OF USING SUBMILISECOND Nd:YAG-LASER RADIATION 1064 NM AND CREAM WITH 1% OF IVERMEKTIN FOR TREATMENT OF PAPULOPUSTULAR FORM OF ROSACEA

Abstract

Rosacea is a polyethiological disease at the base of it progress is pathologically changed innervation of vessels of the skin of face - angioneurosis and it undergoes several stages of development. Initially, against the background of permanent extension, the vascular wall loses elasticity and the blood plasma penetrates it. Microcirculation disorders lead to hypostasis of tissues. This process leads to change of a biota of the face skin - to the uncontrollable growth of number of a Demodex tick, which is one of the ethyopathogenetics factor of promotion from erythemoteleangiectatic form to papulopustular. Etiological reasonable approach for therapy of papulopustular rosacea is a combination of external and physiotherapeutic methods of therapy. As medicamentous therapy for suppression of excess activity of a Demodex tick and its uncontrollable growth in our research we have taken the cream with 1% of Ivermektin. As a physiotherapeutic influence - the submilisecond Nd:YAG-laser radiation (1064 nm). This radiation influences at expanded vessels, sanifies pustules, moderate temperature increase in a derma to 41-42оС, leads to elimination of puffiness and stage-by-stage restructuring of collagen fibers.

ACUPUNCTURE IN ADULT PATIENTS WITH OTITIS MEDIA WITH EFFUSION

Abstract

The basis of conservative treatment of patients with otitis media with effusion is pharmacotherapy which can increased by acupuncture. Under observation were 64 patients aged 20 to 60 years with unilateral otitis media with effusion in the secretory stage, a duration of disease of more than 1 month and the presence of conductive hearing loss. Patients were divided by simple randomization into two comparable groups: group 1 received basic pharmacotherapy within 14 days, group 2 in addition to pharmacotherapy received 7 sessions of acupuncture according to the developed method using of general and local points. The combination of acupuncture with pharmacotherapy increased the effectiveness of pharmacotherapy: by 5.5% in clinical indices, 10.0% - improvement in the otoscopic pattern, 18.1% - in the opening of the ear canal, 8.8% - in the hearing threshold, 17,3% with acoustic muscular reflex, restoration of the barofunction of the auditory tube to the 1 degree by 7.5%, an increase in the immunoregulatory index by 6.1%. In smears-reprints from the mucosa of the mouth of the auditory tube there was an increase in the number of fibroblasts by 14.1%, macrophages by 31.4%; the index of incomplete phagocytosis decreased by 7.1%. In the long term, the frequency of recurrence decreased by 8.6%.

KINESIOTHERAPY IN MEDICAL REHABILITATION OF PATIENTS WITH FOREARM FRACTURES

Abstract

Aims: to study the effectiveness of kinesiotherapy in medical rehabilitation of patients with isolated fractures of the forearm bones. Material and methods. The study was conducted in 44 patients aged 19-85 years with isolated fractures of the forearm bones, who underwent surgical treatment with osteosynthesis. In the control group (n=21), a standard course of medical rehabilitation was conducted in the postoperative period, which included massage, physiotherapy exercises and magnetotherapy. In the main group (n=23), in the postoperative period, the kinesiotherapy of the shoulder and forearm was performed against the background of a standard course of medical rehabilitation. Kinesio tapes were used on the 2nd day after the operation and applied for 72 hours, after which they were changed to new ones. The duration of the study was 12 months. The severity of the pain syndrome, the amplitude of the movement of the wrist joint and the rotational movements of the forearm, the strength of the fist grasp, the function of the upper limb, the quality of life of the patients were evaluated. Results. The use of kinesiotherapy in the period from 1 month after the operation significantly reduced the subjective severity of the pain syndrome by an average of 22.7%, reduced the number of patients with persistent edema by 1.75 times and increased the number of patients without edema by 2.3 times compared with application of a standard course of medical rehabilitation. The parameters of the amplitude of motion in the wrist joint, rotational movements, and the dynamometry of the fist grasp in patients of the main group were slightly higher than those in the control group throughout the observation period. In the long-term period, the number of patients with excellent treatment outcomes in the main group was 1.5 times higher than in the control group, the number of patients with satisfactory results was 1.3 times lower than in the control group. Conclusions. Kinesiotherapy is a promising, simple, non-traumatic method with no side effects and complications, allows to significantly reduce the severity of the pain syndrome, improve the quality of life of patients, contribute to earlier relief of postoperative edema and the most complete restoration of clinical and functional parameters of the wrist joint and can be used as one of the methods in the complex rehabilitation of patients with isolated fractures of the forearm bones.

THE ROLE OF EARLY REHABILITATION AFTER CAROTID ENDARTERECTOMY IN ACUTE PERIOD OF ISCHEMIC STROKE

Abstract

The aim of our study was to evaluate the role of early rehabilitation of patients with ischemic stroke after carotid endarterectomy. Material and methods. We examined a group of 28 patients with atherosclerotic carotid stenosis who had CEA in an acute period of stroke and who started rehabilitation in the early period of stroke. Patients of the 1st group were admitted to rehabilitation in the early stages (within 21 days after the stroke), patients of the 2nd group - in the delayed period (more than 60 days after the stroke).The results of treatment were assessed on the NIHSS scale, Rankin scale, Rivermead mobility scale, spasticity was assessed on the Ashworth scale. Results. An analysis of the long-term results of surgical treatment convincingly showed its positive effect on the neurological and neuropsychological status of patients with the most favorable outcomes in performing early rehabilitation.
