SYSTEMIC THERAPY AND PHOTOTHERAPY OF PSORIASIS - THE POTENTIAL RISKS OF CANCER

Abstract

The relevance of studying psoriasis and its treatment is conditioned by the ever increasing incidence of this condition and frequency of its severe forms. The recent studies on the complications of both psoriasis itself and its treatment have demonstrated the increased risk of associated carcinogenesis. The analysis and systematization of information accumulated to date gives evidence of the relationship between the development of cancer and the application of modern methods for the treatment of psoriasis Moreover, the available data suggest the enhanced risk of cancer in the patients suffering from this disease compared with that in the general population.

THE EFFECTIVENESS OF A NEODYMIUM (1064 NM) LASER AND FRACTIONAL PHOTOTHERMOLYSIS FOR THE CORRECTION OF THE ATROPHIC SCARS

Abstract

One of the challenging problems for modern cosmetology is the cosmetic correction of the cutaneous scars of the skin that arises in the first place from the high prevalence of this condition and the lack of reliable and safe methods for its treatment The application of laser technologies including selective laser therapy as the method of fractional photothermolysis and the use of a neodymium (1064 nm) laser is considered to be a promising approach to address this problem. The present study included 18 patients presenting with hormone-dependent atrophic scars. All the patients were assigned for the treatment by selective photothermolysis (at the rate of 2 sessions per each treatment course performed with a 4 week interval) and irradiation by the neodymium laser (at the rate of 6-8 sessions conducted after fractional photothermolysis). The use of combined laser therapy in patients with hormone-dependent atrophic scars produced a pronounced effect as evidenced by the improvement of the clinical picture and was accompanied by the 78.1% reduction of the symptom index according to the DISC symptom scale and the 76.5% improvement of the dermatology life quality index (DLQI). The use of the combined method for the correction of atrophic cutaneous scars helps to restore the epidermal-dermal and epidermal structures of the skin in the association with the 1.4-fold, 1.7-fold, and 1.6-fold increase in the thickness of the epidermis, dermis, and hypodermis respectively; simultaneously, the acoustic density of the tissues increased by 1.8 times which corresponded to the improvement of the ultrasound picture by almost 75% when estimated in accordance with the Vancouver scar scale.

USE OF PHYSIOTHERAPEUTIC METHODS AT DIFFERENT STAGES OF REHABILITATION OF THE CHILDREN SUFFERING FROM ATOPIC DERMATITIS

Abstract

Atopic dermatitis remains a serious medical and social problem, especially in adolescents. The further development of rehabilitative and physiotherapeutic modalities and their extensive introduction into the clinical practice during the periods of remission make it possible to reduce to a minimum the duration of the oral and topical therapy and thereby to decrease the risk of adverse side effects, improve the quality of life of the patients suffering from atopic dermatitis, and prolong the inter-recurrence periods. The present study included 44 children at the age 15-18 years suffering from atopic dermatitis of mean duration 14.1 ± 1.5 years. They remained under the observation for 48 weeks. The frequency of exacerbations in most of these children (81.8%) was estimated at 4.7 ± 0.8 times a year, with the majority of acute episodes falling on the cold seasons. In 84.1% of the children, the loading coefficient for concomitant somatic diseases was 2.9. All the children received the comprehensive pharmacotherapy and physiotherapeutic treatment in the period of the last exacerbation including transcerebral pulsed electrical therapy, magnetic laser therapy and/or their combination. The study has demonstrated that the use of methods mentioned above during the period of remission in children with atopic dermatitis increased the adaptive potential of the organism, strengthened the general health status of the patients, contributed to the reduction in the frequency and severity of exacerbations of the pathological processes affecting the skin.

THE EFFECTIVENESS OF USING THE MODEL OF PATHOGENETICALLY SUBSTANTIATED REHABILITATION OF SENSORIMOTOR DISORDERS IN THE PATIENTS AFTER ISCHEMIC STROKE

Abstract

The objective of the present study was to evaluate the effectiveness of pathogenetically substantiated rehabilitation of sensorimotor disorders in the patients who had experienced ischemic stroke. Material and methods. A total of 220 patients presenting with hemiparesis following ischemic stroke were available for the examination. The control group was comprised of 35 healthy subjects. All the patients underwent the rehabilitative treatment which included robotized mechanotherapy, stabilography, neuro-muscular stimulation, physical therapy, ergotherapy, massage, etc. Before and after the course of rehabilitation therapy, transcranial magnetic stimulation (MS) was applied and the characteristics of somatosensory evoked potentials (SSEP) were measured in all the patients. Results. Rehabilitation with the use of a personified, pathogenetically grounded approach contributed to a better replenishment of the neurological deficit. In those patients who received the personalized neurorehabilitative treatment significant changes of TMS parameters (central motor conduction time at rest and TMS facilitation test) but not SSEP were registered. Conclusions. The effectiveness of the model of pathogenetically substantiated rehabilitation of sensorimotor disorders in patients after ischemic stroke was confirmed. We have demonstrated a higher effectiveness of restorative therapy of sensorimotor deficit. The somatosensory functions less efficiently responded to the treatment which suggests the necessity of identification of the sensory deficit and taking it into consideration together with neurophysiological monitoring of the rehabilitation process with the use of TMS and SSEP for the enhancement of the effectiveness of ongoing rehabilitation in this category of the patients.

Abstract

The influence of optical radiation of various spectral ranges on the morphofunctional state of the skin under conditions of experimental dermatitis was studied. It was demonstrated that the effectiveness of the light depends on the radiation wavelength. The clinical, histochemical, and morphological data obtained give evidence that the application of blue and red optical radiation to the skin affected by experimental dermatitis is likely to produce the most pronounced therapeutic effect.

Abstract

The authors proposed an original method for the rehabilitation of the patients presenting with disturbances in the oculomotor system including the saccadic eye movements and different types of strabismus associated with the craniocerebral injuries, cranial surgeries and stroke. The most important condition for the achievement of the positive results of the compensatory and restorative processes including the patients’ visual functions, such as binocular vision and their further preservation within the bounds of physiological optimum is the creation and maintenance at an adequate level of the specific motivational dominance in the course of corrective treatment.

FACTORS AFFECTING THE DEMAND FOR RECREATIONAL SERVICES IN THE CAUCASIAN MINERAL WATERS REGION

Abstract

Objective: to study the dynamics of the value orientations of tourists to identify the factors influencing the demand for recreational services in the region of the Caucasian Mineral Waters (CMW). Materials and methods. A method of questioning was chosen for the survey, which was conducted anonymously in 2010-2015 years among holidaymakers of the CMW recreational complex on the following topics: «Satisfaction with the services quality» and «Study of the tourism social aspects». The questionnaires included closed and semi-closed questions. Results. The main purpose of the visit to the CMW region at more than 80% of holidaymakers is a spa treatment and rehabilitation, that is, in the system of values is a concern for health. And only about 15% of visits to the region have other purpose. This suggests that the main specialization of the region of the CMW continues to be a spa treatment, which is a historically prevailing direction of the leisure activities organization for the residents of Russia.

THE EXPERIENCE WITH THE APPLICATION OF THE POLYCHROMATIC INCOHERENT POLARIZED LIGHT FOR THE TREATMENT OF THE PATIENTS PRESENTING WITH ONYCHODYSTROPHIES ASSOCIATED WITH LICHEN RUBER PLANUS

Abstract

This article was designed to report our original results and share the experience with the application of low-intensity polychromatic incoherent polarized radiation for the treatment of the patients presenting with onychodystrophies associated with lichen ruber planus. The clinical effectiveness of phototherapy was evaluated taking into consideration the dermatological index and dynamics of structural changes in the nail. In the patients treated by the medication therapy alone, the dermatological index reflecting the severity of the pathological process decreased by 74% within 4 months after the onset of therapy whereas it fell down by 96% on the average during the same period in the patients in whom the medication therapy was supplemented by the application of low-intensity polychromatic incoherent polarized radiation. It is concluded that the combined pharmacophysiotherapeutic method produces more pronounced effect on the ultrastructure of the nail apparatus than pharmacotherapy alone; its application is pathogenetically substantiated.

THE POTENTIAL OF PREVENTIVE ACUPUNCTURE FOR THE MANAGEMENT OF THE SECONDARY CONTRACTURE OF MIMIC MUSCLES

Abstract

The present article is concerned with the substantiation of the main principles of preventive acupuncture for the management of the secondary contracture of mimic muscles taking into consideration the results of clinical and neurophysiological investigations. We have undertaken the analysis of dynamics of clinical, psychological, and neurophysiological characteristics of the patients following a course of basal treatment and maintenance courses of reflexotherapy. It has been shown that manifestations of the pain syndrome in the patients presenting with the secondary contracture of mimic muscles are consistent with high polysynaptic reflectory excitability. It is recommended, based on the results of this study, to carry out neurophysiological monitoring (for the evaluation of the eye-closure reflex) prior to the prescription of preventive acupuncture for the patients presenting with disturbances of polysynaptic reflectory excitability with a view to obviating the development of a relapse of pain syndrome.

Abstract

This article deals with the problem of physical rehabilitation of the children suffering from infantile cerebral paralysis. It is shown that the process of walking correction in such patients should be focused on the development of the ability to relax skeletal and respiratory muscles and the formation of the novel physical sensations, concurrence and coordination of movements. The author has elaborated the program for the rehabilitative treatment of the children suffering from infantile cerebral paralysis including therapeutic physical exercises and hydrokinesitherapy. The program is designed to promote the enhancement of the muscular strength, the formation of the new locomotor stereotype, and the development of the walking skills.

THE EARLY KNEADING OF THE RADIOCARPAL JOINT WITH A CLOSED FRACTURE OF THE DISTAL RADIUS METAEPIPHYSIS

Abstract

This paper presents a clinical case of the successful treatment of a patient with a closed Association of osteosynthesis type C fracture of the distal metaepiphysis of the left radius. The early three-week rehabilitation using kinesitherapy by the «Artromot H» device within 2 postoperative weeks made it possible to avoid the development of positional stiffness of the radiocarpal joint. Within 6 post-traumatic weeks the full range of motion of the joint and the patient’s working capacity were restored.
