The influence of the factors associated with health resorts on the hormonal regulation of the metabolic reactions in the patients given the surgical treatment for thyroid cancer

Abstract

The present study was designed to analyse hormonal and metabolic changes in the patients given the surgical treatment for thyroid cancer during the rehabilitative period. The experience with the correction of the hormonal regulation of the metabolic reactions gained in the course of the spa and health resort-based treatment is reported with special reference to the application of mineral waters of the different physicochemical composition as the predominant therapeutic factor. It is shown that the resection of the thyroid gland leads to substantial changes in the regulation of carbohydrate and lipid metabolism related in the first place to the altered insulin secretion. The results of the study confirm the effectiveness of rehabilitation of the patients given the surgical treatment for thyroid cancer under the conditions of the North Caucasian spa and health resort facilities. Especially good results were obtained with the use of Essentuki No 4 and Donat magniy mineral waters. It is believed that the mechanisms underlying the therapeutic action of these mineral waters are mediated through the reduction of the severity of insulin resistance syndrome.

Transcerebral UHF therapy of uterine myoma during the early postoperative period: a randomized controlled study

Abstract

We have proposed the scientifically sound substantiation of the therapeutic effectiveness of transcerebral ultrahigh frequency (UHF) treatment following conservative myomectomy. It was shown that this therapeutic modality significantly accelerates regression of the clinical manifestations of this pathology and exerts the well apparent hormone-corrective and psychostimulating action. Moreover, it causes a marked improvement of the psychophysical status of the patients.

Locomotor rehabilitation of the patients suffering from Parkinson’s disease

Abstract

The importance of the development of the methods for the combined medical rehabilitation with the use of non-medicamental technologies designed to improve the social and living adaptation and the quality of life of the patients presenting with Parkinson’s disease (PD) arises from the absence of the programs for their longterm combined locomotor rehabilitation. In this context, the development of such programs appears to be a top-priority objective. It can be achieved with the use of the methods currently applied in clinical practice for the improvement of the locomotor functions, such as therapeutic physical exercises, treadmill training, Nordic walking, tempo-rhythmic correction of the gait, dance therapy, Tai Chi gymnastics, and ergotherapy.

The application of the non-medicamental technologies for the early rehabilitation of the patients presenting with cerebral ischemic stroke

Abstract

A total of 102 patients presenting with cerebral ischemic stroke were available for the examination. They were treated with the use of the immersive reality system (BTS Nirvana) and the KOBS multi-functional training platform. It was shown that the proposed therapeutic modalities make it possible to reduce the severity of manifestations of neurological deficiency as estimated based on the NIHSS scale and that of locomotor disorders evaluated from the enhancement of the muscular force.

Transcranial physiotherapy in the patients presenting with chronic fatigue syndrome

Abstract

The results of examination of 100 patients presenting with chronic fatigue syndrome are reported. The method for the correction of this pathological condition based on the combination of transcranial physiotherapy (CTcPhT) [magnetotherapy plus transcranial electrical stimulation (TES) therapy] is described. The study has demonstrated that the application of this method as a monotherapeutic modality makes it possible to eliminate the signs of vegetative dysfunction (VD) and disadaptation in more than a half of the patients.

The application of decimeter wave therapy for the combined treatment of destructive appendicitis and its complications in the children during the postoperative period

Abstract

Detailed analysis of the intraoperative picture in 534 patients enabled the authors to develop the method for decimeter wave therapy (DWT) and introduce it into the combined treatment of the children presenting with destructive appendicitis after they underwent laparoscopic appendectomy. The effectiveness of the proposed differential approach to physiotherapy was based on the results of a comparative study of the patients’ conditions during the postoperative period with the use of DWT and without it. The study has demonstrated the advantages of DWT therapy given during postoperative period after the surgical treatment of destructive appendicitis in the children. It was found that the prescription of DWT therapy to the patients with appendicular peritonitis starting from the first day after the surgical intervention reduces the frequency of syndrome of systemic inflammatory reaction (SSIR) and intestinal insufficiency syndrome (IIS). Moreover, DWT was shown to accelerate the reversal of the latter syndrome. Its duration in 48% of the patients decreased down to 1.8 ± 1.2 days in comparison with 2.3 ± 1.3 days in 58.5% of the patients given the combined treatment without DWT. Syndrome of systemic inflammatory reaction was documented in 43.0% and 55.7% of the patients treated with the use of DWT and without it respectively, with its duration being 2.6 ± 1.6 and 4.9 ± 1.8 days (p < 0.05). In addition, the rapid elimination of the local inflammatory process under the action of DWT therapy was confirmed by the results of the tomographic examination of the anterior abdominal wall in the right ileal region. The patients treated with the use of DWT therapy experienced a two-fold drop in the absolute body temperature and temperature gradient within 5 days of the postoperative period.

The experience with the organization of rehabilitation of the patients presenting with neurosurgical pathology under the conditions of a specialized clinical centre

Abstract

The analysis of the second and third levels of evidence has demonstrated the necessity of the early rehabilitation of the patients presenting with diseases and lesions of the nervous system. It is estimated that on the whole from 50% to 75% of the patients in need of neurosurgical and neurological aid require the rehabilitative treatment in one or another form. The onset of the rehabilitative measures in the early period of the disease depends on the character of neuroplastic processes that are especially active in the acute phase of the disease. Early rehabilitation increases the probability of preventing of its unfavourable outcome.The objective of the present study was to enhance the effectiveness of neurological rehabilitation in the patients presenting with neurosurgical pathology. We analysed the results of the rehabilitative treatment given to the patients admitted to A.L. Polenov Russian Neurosurgical Institute, Russian Ministry of Healgth, during the period from 2010 till 2013. The study has demonstrated that active rehabilitation including the preoperative preparation, intraoperative procedures, and combined application of the rehabilitative techniques in the acute phase of the disease makes it possible to achieve as high therapeutic efficiency as 86.7% despite the short duration of the rehabilitation courses. The practically complete clinical recovery was documented in 6% of the patients, the marked improvement of their health status in 38.2%, and the apparent improvement in 42%. It is concluded that the active rehabilitation and the combined physiotherapeutic treatment (PT) at different stages of the disease including the preoperative preparation and intraoperative procedures makes it possible to enhance the effectiveness of therapeutic modalities in the patients presenting with neurosurgical pathology.

The age-related characteristics of the medical aid appealability of the adult population

Abstract

This paper reports the results of the estimation of the requirements in different age-groups of the adult population for medical rehabilitation based at the outpatient facilities (OPF) calculated from the analysis of the medical assistance appealability of the adult population seeking the rehabilitative care depending on the main classes of pathology and a concrete disorder in accordance with the International Classification of Diseases (ICD-10). For the purpose of this study, the adult population was categorized into 6 age-groups, viz. 18 - 29, 30 - 39, 40 - 49, 50 - 59, 60 - 69, and 70 years and more. The healthcare providers and experts have developed the indications for the rehabilitative treatment starting immediately after the management of acute conditions and treatment of exacerbation of chronic diseases, consequences of the injuries and surgical interventions taking into consideration the stage of pathology and the patients’ health status. In addition, the conditions necessary to treat such patients based at inpatient or outpatient facilities are determined. Taking account of the data thus obtained for the analysis of the medical documentation made it possible to make a decision about the necessity and/or duration of further outpatient rehabilitation of the patients following their inpatient treatment. We estimated and analysed the age-related requirements of the adult population in the medical assistance and calculated the number of the outpatient visits depending on the main classes of pathology and a concrete disorder. It was shown that the subjects of the advanced age (60-69 and 70 or more years) make up the group of those who are most in need of medical care (3917.5 and 4716.9 visits per 10,000 population respectively) in comparison with the group of 18-29 year old subjects (832.2 visits per 10,000 population). Evidently, this difference can be accounted for by the higher frequency of diseases as well as the enhanced requirements for inpatient rehabilitation among the older age groups. The bodies of executive power in the subjects of the Russian Federation can calculate the requirements of the local adult population in the rehabilitative treatment based at the outpatient facilities for different age groups and regardless of the territory of the region.

For the purpose of this study, the adult population was categorized into 6 age-groups, viz. 18-29, 30-39, 40-49, 50-59, 60-69, and 70 years and more. The healthcare providers and experts have developed the indications for the rehabilitative treatment starting immediately after the management of acute conditions and treatment of exacerbation of chronic diseases, consequences of the injuries and surgical interventions taking into consideration the stage of pathology and the patients’ health status. In addition, the conditions necessary to treat such patients based at inpatient or outpatient facilities are determined. It was shown that the subjects of the advanced age (60-69 and 70 or more years) make up the group of those who are most in need of medical care (874,7 and 1346,3 per 1,000 population respectively) in comparison with the group of 18-29 year old subjects (106,2 per 1,000 population). Evidently, this difference can be accounted for by the higher frequency of diseases as well as the enhanced requirements for both inpatient and outpatient rehabilitation among the older age groups. The bodies of executive power in the subjects of the Russian Federation can calculate the requirements of the local adult population in the rehabilitative treatment for different age groups based on the above data on the frequency of hospitalization.
