The transcapillary brain metabolism in the patients presenting with the injury to the mid-facial skeleton

Abstract

The present paper reports the result of the investigation into the state of the transcapillary brain metabolism in 110 patients presenting with the injury to the mid-facial skeleton, It was shown that all these patients suffered the impairment of transcapillary brain metabolism before the onset of the treatment that is in all probability characteristic of the pathology in question and provides a basis for the development of organic and psychological complications. The traditional combined treatment of the patients comprising the second group was supplemented by the local and general magnetic therapy as well as multi-channel electrical stimulation. Only these patients responded to the treatment by the improvement of the transcapillary brain metabolism and general clinical conditions. The statistical analysis of the data obtained allowed to make the short-term positive prognosis for index F (up to 14 months).

The influence of different programs for the combined rehabilitative treatment of certain main clinical syndromes in the patients presenting with atherosclerosis of peripheral arteries in the lower extremities

Abstract

The present paper reports the result of the implementation of the integrated program for the rehabilitative treatment of the patients presenting with atherosclerosis of peripheral arteries in the lower extremities including supravenous blood laser irradiation, application of the traveling magnetic field to the collar region and gastrocnemius muscles, dry carbon dioxide baths alternating with horse chestnut baths. The study involved 200 patients with the above pathology. It gave evidence of the highly effective correction of abnormalities in the lipid spectrum and restoration of normal metabolic parameters. Moreover, the rehabilitative program ensured the well-apparent vasocorrective effect of primary importance for the management of the pathology being considered.

The combined rehabilitative treatment of the patients suffering from the consequences of acute disturbances to cerebral circulation

Abstract

The application of the pathogenetically substantiated rehabilitative programs permits to reduce the severity of focal neurological symptoms and the degree of cognitive disorders, improve cerebral hemodynamics as well as tonic and strength characteristics of the paretic muscles. Moreover, such approach to the rehabilitation including the application of nicotinic acid electrophoresis exerts the positive influence on the rheological parameters of blood and lipid peroxidation; these effects promote prevention of repeated stroke. The socio-economic effectiveness of the use of combined rehabilitative technologies during the period of possible manifestations of the consequences of acute disturbances to cerebral circulation is apparent as the enhancement of the everyday activity of the patients and the approximation of their quality of life to the pre-stroke level.

Abstract

We have developed the method for the combined rehabilitative treatment of the patients presenting with osteoarthrosis following total hip endoprosthetic replacement during the late postoperative period with the application of the natural and pre-formed physical therapeutic factors. The method was designed to reduce to a minimum the manifestations of the clinical symptoms and the activity of inflammation, to optimize the functioning of the immune, lipid peroxidation, and antioxidative prevention systems, to activate the metabolic processes in the articular and peri-articular tissues, to stimulate the adaptive capabilities of the body, and to increase the strength of the muscles responsible for the movements in the affected joints and thereby to improve the motor activity of the patients and the quality of their life.

The pathogenetic substantiation of the neurostimulating treatment of metabolic syndrome in the patients presenting with erectile dysfunction

Abstract

Pharmacotherapy of erectile dysfunction in the patients presenting with metabolic syndrome is known to frequently produce the unstable effect of short duration because it fails to exert the polyvalent influence on the main regulatory systems. The investigations carried out during the recent years have demonstrated that the most pronounced therapeutic effect in the patients with metabolic syndrome can be achieved by applying the physiotherapeutic factors affecting all components of pathogenesis of this disease. Of great interest in this context is the use of neurostimulating therapy that exerts the positive influence on the cardiovascular and neuroendocrine systems, carbohydrate and lipid metabolism. Moreover, this approach normalizes the vegetative regulation and the erectile function. Our study included 40 patients presenting with metabolic syndrome and concomitant erectile dysfunction. All of them were treated with the use of neurostimulating therapy. It was shown that electrical stimulation of cavernous nerves and muscles of the pelvic floor improved the quality of erection, stimulated the orgasmic and ejaculatory functions, activated blood circulation in the tissues of the urogenital system, and exerted the anti-inflammatory action as well as produced the regenerative effect. It is concluded that the above changes can be accounted for by the partial restoration of vegetative and somatic innervations of the penis. This inference was confirmed by the results of repeated investigations.

The role of combined physiotherapy in the treatment of the persistent forms of Chlamydial prostatitis complicated by erectile dysfunction

Abstract

The present study included 52 patients at the mean age of 37.5 years presenting with chronic prostatitis (CP) associated with Chlamydia trachomatis infection. It has demonstrated the effectiveness of the method based on the combination of systemic and local antibacterial and physiotherapeutic treatment with the administration of proteolytic enzymes into the prostatic gland region with the use of the AMUS-01 - “INTRAMAG” instrumental complex. The participants of the study had the history of erectile dysfunction (ED). It necessitated the application of local negative pressure (LNP) therapy with the use the “AIR-U plus” apparatus in addition to local physiotherapy. The immune status of the patients treated in accordance with the described protocol was evaluated based on the expression of the cytokine-encoding genes influencing the populations of T-lymphocytes and phagocytes. The serum interleukin levels were determined by means of the solid-phase immunoenzymatic assay with the use of the commercial kits manufactured by “Tsitokin” Ltd. (Sankt-Peterburg). The data thus obtained were compared with the results of the analogous measurement in the group of healthy volunteers (n = 10). The control subjects received the combined treatment including the medicamental correction of microcirculation and inflammation making use of non-steroidal anti-inflammatory agents, enzymatic preparations, immunomodulators, and herbal medicines for the stimulation of the penile blood flow. This treatment was supplemented by conventional physiotherapy in the form of low-intensity infrared laser irradiation of the perineal region. The duration of a single course of combined treatment was 15 days. The proposed strategy for the systemic and local antibacterial treatment in combination with physiotherapy and the use of the AMUS-01“INTRAMAG” instrumental complex permitted to increase the effectiveness of eradication of Chlamydia by 18,3%. Local negative pressure therapy initiated after each session of the instrumental treatment made it possible to additionally improve the immunological characteristics of the patients as well as the results of laboratory studies of the secretion, the volume of the prostatic gland, the erectile function and the quality of life of the patients by a factor of 2.8 in comparison with 1.6 in the control group.

The possibilities for the use of mineral water in the combined treatment of acute maxillary sinusitis

Abstract

The present article was designed to report the results of the investigations into the mucociliary activity of mucosal epithelium in the nasal cavity and the local immunity status of the patients presenting with acute maxillary sinusitis during their combined treatment with the use of Talaya nitrous siliceous water as elimination therapy and endonasal electrophoresis. The study included 120 patients with acute suppurative maxillary sinusitis and 15 health subjects. The patients of group 1 (control, n = 48) were given standard medicamental therapy and underwent the maxillary sinus puncture followed by the administration of an antibiotic. The patients of group 2 (n = 72) received the standard medicamental treatment with the puncture of the maxillary sinuses into which mineral water was introduced; this procedure was supplemented by mineral water electrophoresis in the nasal cavity using the specially developed method. The effectiveness of the treatment was assessed based on dynamics of clinical symptoms, results of rhinoscopy and X-ray studies of the paranasal sinuses. In addition, the transport function of ciliary epithelium was evaluated from the results of the saccharin test and the examination of the nasal mucus smears. The lavage from the maxillary sinuses obtained during their puncture or with the help of the sinus-catheter were used to determine concentrations of albumin, immunoglobulins IgA, M, G, E, and secratory sIgA. The study has demonstrated that the clinical effectiveness of the treatment with the use of mineral water as elimination therapy in combination with endonasal electrophoresis was attributable to the improvement of mucociliary clearance , reduction of the inflammatory reaction, and correction of the local immune protection factors. The duration of the mucociliary transport in the patients of groups 1 and 2 decreased by 39.8% and 44.2% respectively. The analysis of rhinocytograms revealed a significant decrease in the number of phagocytizing neutrophils, the enhancement of autoflora phagocytosis, the decrease in percentage of destroyed phagocytes, and the increase in the number of lymphocytes. The comparative analysis of dynamics of the local protective factors has demonstrated the advantages of the treatment with the use of mineral water as well as significant differences in the degree of reduction of IgA, sIgA, IgM, and IgE levels. It is concluded that the results of this study provide a basis for the introdution of mineral water in the combined treatment of the patients presenting with acute suppurative maxillary sinusitis and the extension of the scope of physiotherapeutic methods for the treatment of this pathology.

The recreational potential of the Yangan Tau health resort

Abstract

Our studies have demonstrated gradual depression of the temperature regime of geothermal process in the interior of the Yangantau mountain at a rate of 0.5 - 1.0 degrees per year. This process is a result of a rare combination of geographic, climatic, and geochemical conditions giving rise to a thermodynamic system in the form of a natural reactor. This system is characterized by highly effective thermolysis of the organic mass contained in bituminiferous shales with the production of the thermal gas-vapour mixture that serves as the main therapeutic factor utilized at the Yangan-Tau health resort. However, the limited resources of bituminiferous shales in the interior of the Yangantau mountain tend to be exhausted. Therefore, they need to be replenished with the products of bituminiferous shale thermolysis from other locations if the geothermal process in the interior of the Yangantau mountain to be prolonged.

Balneotherapy of arterial hypertension with the use of sodium chloride mineral waters

Abstract

The present review of the current literature concerns the problem of balneotherapy with the use of sodium chloride mineral water (SCMW) for the treatment of the patients presenting with arterial hypertension (AH). The principal component in the primary mechanism of action of these baths is related to the specific effects of mineral salts resulting in the formation of the “salt-coat” at the skin surface that causes persistent irritation of its receptor apparatus with the subsequent reflectory influence on the entire body conditions and its thermoregulation processes. The mineral sodium chloride baths with a salt concentration of 20-40 g/l produced the hypotensive effect in the patients suffering from AH. In addition, they promoted the correction of systemic and cerebral hemodynamics, improved the vegetative regulation of the cardiovascular system, lipid metabolism, and the morphofunctional state of the microvessels. Large natural resources of sodium chloride mineral waters and their widespread distribution coupled to the possibility of their artificial production create the prerequisites for the excellent prospects of the extensive application of this balneological factor in the clinical practice.
