The principles of staged rehabilitation of the patients presenting with a severe injury to the spinal cord

Abstract

The article features the practices related to the multi-faceted rehabilitation of patients presenting with the severe combined injuries that affect both the spine and the spinal cord. The issues of interest in terms of the immediate post-operative rehabilitation as well as the experience gained during the treatment of major locomotor disorders are defined and discussed. The article presents the guidelines concerning the development of the rehabilitation programs and the major drug-free rehabilitation strategies including those based on the application of the up-to-date robot-assisted technologies. The training devices and other instruments of the domestic and foreign production being currently put into use are considered along with the practical guidelines designed to facilitate decisions making as regards the prescription of one or another treatment modality taking into account the relevant indications. It has been found that the precise body weight support systems efficiently help to improve walking performance through unloading of the patient’s body which is conducive to the sensory stimulation that best fits the concrete patient. The advantage of robot-assisted rehabilitation consists of the possibility to do training in the sub-maximum regimen that enables the patient to carry out the exercises controlled in terms of both their overall number and intensity level. The immediate post-operative rehabilitation aims primarily at the enhancement of motor performance by allowing the patient to develop the ability to stand upright within the shortest time possible. it would take roughly 20-40 days for a patient to adapt to standing upright in the absence of the specialized training devices whereas the use of the tilt tables with built-in robotic continuous passive and active motion devices for the training of the lower extremities eliminates a number of factors impeding rehabilitation of the patients and thereby makes it 2-3 times faster for a patient to adapt to standing upright. The suspension systems have been found to be promising for the rehabilitation of the patients experiencing the locomotor problems following a severe injury to the spinal cord.

The effectiveness of rehabilitation of the patients presenting with the consequences of a vertebral and spinal cord injury with the use of the functional electrical stimulation

Abstract

The vertebral and spinal cord injury is considered to be an emergency condition that leads in the majority of the cases to persistent disability. Hence, the necessity of restoration of the functional capacity of the spinal cord. We have undertaken the evaluation of the results of the spa and health resort-based treatment of 88 patients presenting with the consequences of a vertebral and spinal cord injury in the thoracolumbar vertebral column. The age of the patients ranged from 18 to 62 years, the duration of the disease varied from 1 to 10 years. The study has demonstrated the high effectiveness of the proposed comprehensive rehabilitation program apparent as the considerable increase of the muscular force, marked improvement of the daily living and locomotor activities, trophic functions and those of pelvic organs as well as of the general psychological status of the patients. It has been shown that the inclusion of functional electrical stimulation in the program of the combined rehabilitative treatment of the patients with vertebral and spinal cord injury facilitates the improvement of tactile sensitivity and functions of the pelvic organs. This beneficial effect of comprehensive rehabilitation was documented from the results of the evaluation based on the ASIA scale in the patients of group A.

The selected results of and the prospects for the modification of drinking mineral waters differing in the degree of mineralization

Abstract

The possibility to enhance the biological potential of drinking mineral waters is of great theoretical and practical significance. The authors of the present publication undertook an attempt to address this issue based on the analysis of the experience of domestic and foreign researchers. The article presents the results of clinical and experimental investigations of mineral waters and their combinations with various pharmaceutical products, herbal raw materials, and dosage forms of herbal medicines, silver and selenium nanoparticles. The modifying additives contained various groups of biologically active compounds responsible for their activity and were used for the correction of metabolic disorders associated with a variety of diseases. The results of the study provide a basis for the development of the rationale for the elaboration of the novel therapeutic and prophylactic algorithms for the application of drinking mineral waters in the combination with the aforementioned modifying additives.

The effectiveness of the method of extracorporeal shock wave therapy for the treatment of the patients presenting with degenerative-dystrophic lesions of the tendons, muscles and ligaments of the shoulder joint

Abstract

The objective of the present study was to evaluate the effectiveness of the method of extracorporeal shock wave therapy (ECSWT) for the treatment of the patients presenting with degenerative lesions of various severity in the tissues and anatomical structures involved in the formation of the shoulder joint. The study and the analysis of its results were carried out based at the “Vulan” Spa and Health Resort Facility. A total of 34 patients at the age of 30-68 years presenting with persistent pain in the projection of the shoulder joint were treated during the period of 12 months by means of ECSWT. The results of their treatment were compared with those obtained in the management of 37 patients with a similar pathology managed using the traditional therapeutic modalities, such as phonophoresis with hydrocortisone and ozokerite therapy. The treatment was carried out with the use of a “Shockmaster 500” (Germany) apparatus, equipped with the “Karl Storz” (Switzerland) applicators providing for the radial spread of the shock wave. The range of the active and passive movements in the joint before and after the course of therapy was evaluated using the “Con Trex” robotic complex (Switzerland) based on biological signal feedback using the Swanson scale score for the shoulder (the Swanson shoulder score, 1989) and the Likert scale. The analysis of the results confirmed the significant improvement in the condition of the treated patients as compared with that in the control group. Specifically, the range of the active and passive movements in the affected joint increased whereas the intensity of the pain syndrome was either relieved or it was completely eliminated. As a consequence, the quality of life of the patients was markedly improved. These findings give grounds to recommend the extension of the application of the ECSWT-based treatment at various stages of rehabilitation of the patients suffering from humeroscapular pain for the purposes of remedial medicine and clinical practice.

The influence of magnetic therapy and naftalan treatment on the clinical course of the gastroesophageal reflux disease with the concomitant bronchial asthma

Abstract

The objective of the present study was the elaboration of the scientifically grounded rationale for the development of the technology of application of the low-frequency alternating magnetic field for the treatment of the patients presenting with gastroesophageal reflux disease and the concomitant bronchial asthma. Materials and methods. We undertook the comparative evaluation of the effectiveness of the rehabilitative therapeutic modalities and their influence on the course of gastroesophageal reflux disease in 47 patients with the concomitant bronchial asthma. The study was designed to carry out the clinical examination of the patients and verification of the diagnosis, random formation of the study groups, and their control studies with special reference to the clinical symptoms based on the results of esophagogastroduodenoscopy, chest X-ray, the study of external respiratory function, analysis of sputum, and assessment of inflammation. Results. It has been demonstrated that the low-frequency alternating magnetic field in the combination with the local applications of naftalan has the beneficial influence on the clinical course of gastroesophageal reflux disease with the concomitant bronchial asthma. The proposed combined treatment modality made it possible to improve the clinical symptoms of the composite pathology and resulted in the reduction of the inflammatory activity as appeared from the decreased amount of eosinophils, reduced fibrinogen, anti-inflammatory cytokines, tumour necrosis factor, and interleukin-1p levels in the blood, normalization of the physical properties and microscopic characteristics of the sputum, and the improvement of the external respiratory function.

The effectiveness of laserophoresis of the autologous plasma with the blood cells at different morphological types of involutional skin changes

Abstract

The present study was designed to evaluate the efficiency of the application of the autologous plasma containing blood cells in the combination with low-intensity laser radiation for the treatment of the patients presenting with different morphological types of involutional changes in the skin depending on the effectiveness of the microcirculation processes. The impact of different morphological types of cell aging in the light recovery apparatus was compared. The technologies influencing the vascular component used in the study included low-intensity laser irradiation in the combination with laserophoresis of the autologous plasma containing blood cells.
