THE PROSPECTS FOR THE FURTHER DEVELOPMENT OF THE ROBOTIC MANIPULATION MECHANOTHERAPEUTIC COMPLEXES

Abstract

The ongoing active development of robotics in the framework of the new economic paradigm known as «the 4th technological revolution» opens up new possibilities for the progress in various disciplines including medicine. The further improvement and clinical application of robotic manipulation mechanotherapeutic complexes are among the major priorities for the Russian public healthcare system. The article represents the characteristics of the Russian robotic manipulator prototype designed to enhance the effectiveness of medical rehabilitation, develop various options of the robotic situational behavior, and specify the topical issues as regards the improvement of the manipulation robotic systems that require the effective solutions and actions. The prospects for the formation of the basic principles of new specialty «massage programmer» are outlined based on the above considerations.

THE DEFINITION OF THE TERM OF «VIRTUAL REALITY» IN THE CONTEXT OF MEDICAL REHABILITATION

Abstract

The extensive application of various types of biological feedback mechanisms for the purpose of medical rehabilitation, e.g. as exemplified by «immersion» of the patient in the synthetic («virtual», «enhanced») environment, poses an important problem concerning the terminology, definitions, identification of the differences, and the selection of such artificial realities. In the current Nomenclature of medical services introduced into the clinical practice by the Order of the Ministry of Health of the Russian Federation (No 1664n dated 27.12.2011), the following codes can be attributed to the items concerning the possible use of virtual environments: A19.03.001.014 «Training with biological feedback from the support reaction in patients presenting with spinal injury» or A19.03.001.017 «Training with biofeedback via the kinesiology image of the manner of motion in the patients suffering from spinal injury», etc. At present, the use of the virtual environments and objects for the purpose of relaxation which are yet not coded as the medical services is extensively practiced at the treatment-and-prophylactic establishments of the spa and health resort type. This brief review is perhaps the first or one of the first attempts to explicitly define and represent the multidisciplinary approach to the problem being analyzed in the context of medical rehabilitation with the consistent information synthesis. The aim of the present work was to provide rehabilitation medicine professionals and other specialists with the information pertaining to the interpretation of the materials having a bearing on the main line of these studies.

Abstract

At present, degenerative dystrophic disease of intervertebral disks and lumbar back pain associated with this condition constitute a most serious medical, social and economic problem. This article was designed to consider underwater spine traction as a drug-free treatment for low back pain. The literature review includes the historical milestones and current concepts of traction therapy. The paper includes the analysis of the long-established clinical notions having the influence on the evaluation of the effectiveness of the treatment and proposes the new up-to-date approach to the investigation of the pathological condition being condidered and the method for its therapy.

REHABILITATION OF THE PATIENTS AFTER THE SURGICAL TREATMENT OF HERNIATED INTERVERTEBRAL DISCS

Abstract

The objectives of the study was to enhance the effectiveness of the rehabilitation of the patients who have undergone the surgical intervention on the herniated intervertebral discs in the lumbosacral spine with the use of general magnetic therapy during their combined treatment. A total of 67 patients were available for the medical examination and treatment. The patients presenting with similar clinical symptoms were allocated to two groups matched for age and gender. All of them received initial therapy including medication, kinetic physical therapy, and hydrokinesitherapy. The patients of the main study group were additionally given magnetic therapy while those in the control group were treated without additional therapy. It has been shown that the introduction of general magnetic therapy into the program of the combined treatment exerts the beneficial influence on the main clinical symptoms of the disease, intensifies the regeneration processes, and improves blood circulation in the surgical intervention region; moreover, it improves the psychoemotional condition and the quality of life in this category of the patients.

THE MODERN METHODS FOR THE CORRECTION OF NON-NEOPLASTIC MELANIN HYPERPIGMENTATION OF THE SKIN AND THE INTEGRATED APPROACH TO THEIR TREATMENT

Abstract

Skin hyperpigmentation is one of the most common conditions that attract the attention of both the dermatologists and the cosmetologists since the cosmetic defects associated with this pathology are known to greatly deteriorate the quality of life of the patients. This article presents information about the most popular modern methods for the correction of non-neoplastic melanin hyperpigmentation of the skin. The main trends in the application of the external agents, laser and light therapy as well as the possibility of the combination of some of these techniques are considered. Methods: A total of 24 patients presenting with various clinical forms of non-neoplastic melanin hyperpigmentation of the skin aged from 25 to 62 years were under supervision in the present study. The pathological process was mainly localized on the face, the neckline and the back of the hands in 21 (87.5%) patients, on the skin of the trunk and the extremities in 3 (12.5%) ones. All the patients received topical therapy with the use of the products containing glycolic acid and ascorbic acids in the form of both drug monotherapy and its combination with laser therapy (at a wavelength of 1064 nm). Results: The study has demonstrated that the topical application of the products containing glycolic acid and ascorbic acid as a monotherapy provides a highly efficacious and safe method for the aesthetic correction of skin hyperpigmentation. However, its combination with laser therapy (at a wavelength of 1064 nm) produces a significantly higher therapeutic effect as confirmed by the present study.

PHOTOHEMOTHERAPY IN THE COMBINED TREATMENT OF THE PATIENTS PRESENTING WITH BRONCHIAL ASTHMA

Abstract

The article presents the results of the application of photochemotherapy for the combined rehabilitation treatment of the patients suffering from bronchial asthma. The assessment of the effectiveness and the impact of this treatment was made on the basis of changes of the indicators of blood rheology as one of the components of pathogenesis of this disease with due regard for the signs and symptoms characterizing the clinical course of this condition. It has been demonstrated that 74 patients presenting with bronchial asthma experienced the statistically significant restoration of the altered rheological parameters of blood and the general improvement of the clinical course of the disease under the influence of photochemotherapy in comparison with the results of the baseline treatment obtained in the remaining 42 patients. The effect of optical radiation of blue and red light on the clinical course of bronchial asthma was more pronounced than that of the conventional treatment. The two therapeutic modalities were virtually identical when evaluated based on the effectiveness criteria, but differed in terms of the mechanism of action on the body.

THE APPLICATION OF WAVE BIOMECHANICAL THERAPY FOR THE COMBINED TREATMENT OF THE PATIENTS SUFFERING A SNAKE (VIPER) BITE DURING THE REHABILITATION PERIOD

Abstract

We have undertaken the comparison of the results of the treatment and the changes in the laboratory characteristics of 25 patients that suffered a viper bite and were given the combined treatment either with the application of wave biomechanical therapy (WBMT) (10 patients comprising the study group), and without it (15 patients making up the control group). During the rehabilitation period, all the patients exhibited the common symptoms of intoxication including fever, shortness of breath, dizziness, nausea, dilated pupils, and rapid heartbeat as well as the local changes (burning sensation, redness of the skin at the site of the bite, decreased skin sensitivity, swelling, bruising, and blisters); other pathological conditions documented in the association with the snake bite included lymphangitis, lymphadenitis of the regional lymph nodes, and extensive swelling of the extremities with severe pain which were collectively responsible for the significant prolongation of the duration of hospitalization and required an additional treatment. The laboratory studies have demonstrated the multidirectional disorders of the hemorheologic patterns and of the cellular component of toxemia, such as deviations from the normal values of practically all hemorheological characteristics, mostly in the upward direction, moderate leukocytosis, and the increase in the amount of the dead white blood cells, as well as the significant enhancement of the indicators of endotoxemia including the leukocyte index of intoxication, and the coefficient of endogenous intoxication (a 1.8-5.7 times elevation, respectively). The application of the WBMT technology gave rise to positive clinical and laboratory dynamics (more pronounced in the patients of the study group than in those of the group of comparison) in the form of the reverse development of the general manifestations of intoxication and, especially, such local changes as reduced swelling, skin redness, bleeding, feelings of pain, normalization of body temperature and heart rate, restoration of skin sensitivity. The use of WBMT was accompanied by a full and pronounced correction of hemorheological disorders (viscosity and aggregation), and the 1.5-fold higher frequency of their effective correction in the treatment group compared with that in the control patients. There was an apparent tendency towards the normalization of endogenous intoxication and higher overall contents of the blood fractions of medium molecular weight peptides (MMP) (with the 1.2 and 1.4 times reduction in the levels of the MMP254 and MMP280 fractions, respectively) while leukocyte index of intoxication showed a statistically significant fall by a factor of 3.5 in comparison with its initial value. It is concluded that the introduction of WBMT in the combined therapeutic modality makes it possible to decrease the duration of hospitalization of the patients by 1.5 times (33%) on the average.

THE EXPERIENCE WITH THE APPLICATION OF THE «EKZARTA» (REDCORD) KINESIOTHERAPEUTIC EQUIPMENT FOR THE REHABILITATION OF THE PATIENTS WITH SPINAL CORD PATHOLOGY AT THE CERVICAL LEVEL

Abstract

The patients suffering a traumatic spinal cord injury (TSCI) at the cervical level need motor rehabilitation the adequate achievement of which is hampered due to the development of tetraparesis up to and including tetraplegia. For this reason, they require creation of the special conditions for making the prescribed physical exercises. The objective of the present study was to evaluate the influence of exercises performed with the use of the “Ekzarta” (Redcord) kinesiotherapeutic equipment on the neurological status of the patients with spinal cord pathology at the cervical level. A total of 102 patients referred to group 1 disability were available for the observation before, during the intermediate and late rehabilitation periods after the traumatic spinal cord injury at the cervical level representing the A, B, C, and D neurological types as estimated based on the ASIA/ISCSCI scale. The examination included gathering complaints, assessment of the neurological status with the evaluation of the motor, manual, locomotor functions, estimation of the inferior segment with normal sensitivity, and the presence of sensitivity in the paralyzed parts of the body. Spastic syndrome intensity was assessed based on the Ashworth scale, and the intensity of pain syndrome with the application of the visual analogue scale. The treatment resulted in the elevation of arterial pressure, reduction of the intensity of spastic and pain syndrome, and significant increase of the motor scores. The statistically significant increase of the locomotor scores was also documented in the patients with types C and D neurological deficit while the patients with neurological deficit of types A, B, C exhibited the significant increase of the manual scores.
