Doctors in the eyes of the relatives of stroke patients: mutual understanding and relationship

Abstract

The relatives of the patients presenting with acute cerebral circulation problems (ACCP) and admitted to a neurological department can form the opinion about the quality of the doctor’s work only based on the patients’ health status without regard for the professional skills and dedication of a given specialist; this fact can be attributed, among other things, to stress (a total of 76 subjects were interviewed). At the same time, those understanding the specific character of the medical profession are capable of highly appreciating the quality of the doctor’s work even in the case of an unfavourable outcome of the treatment. All the respondents who participated in the present study emphasized the necessity of the efficient communication with the attending doctor that is in our opinion one of the most important conditions indispensable for the creation of the normal working environment and the reduction of the psychic tension. The authors discuss the possible ways of cooperation between the doctor and the patient’s family with the purpose of reducing stress on the relatives, improving the quality of care and rehabilitative measures after the patient’s discharge from the hospital, and preventing the loss of emotional sensitivity by the doctors.

The quality of life of the patients suffering from bronchial asthma treated by the combination of hypoxically hypercapnic stimulation and amplipulse therapy as a component of the spa and health resort-based treatment

Abstract

The treatment of the patients presenting with bronchial asthma (BA) has the purpose of achieving the total control over the disease and improving the quality of the patients’ life. The objective of the present study was to elucidate the influence of hypoxically hypercapnic stimulation (HHS)and amplipulse therapy (APT) or their combination on the quality of life (QL)of the patients suffering from bronchial asthma. The study included 127 patients with BA examined before and 6 months after the spa and health resort-based treatment. The quality of life was estimated with the use of the MOS 36-Item Short-Form Health Survey questionnaire. HHS and APT procedures were carried out in combination with the comparable pharmacotherapeutic an balneotherapeutic treatment provided In the healthcare facilities located at the South Coast of Crimea (Yalta). It was shown that the application of hypoxically hypercapnic stimulation and amplipulse therapy, separately or in the combination, makes it possible to improve the quality of life of the patients suffering from bronchial asthma as demonstrated by their increased physical strength, viability, social activities, and psychic health as a whole. Moreover, such patients show more positive subjective attitude toward their own general health status. The most beneficial improvement of all characteristics of the quality of life was documented in the case of the combined application of hypoxically hypercapnic stimulation and amplipulse therapy. Specifically, the physical activity of the patients increased from 58.83 ± 18.44 to 76.12 ± 14.48 scores (p<0,05), the occurrence of the physical and psychic problems decreased as apparent from the increase of the relevant indices from 22.63 ± 24.66 to 56.24 ± 20.48 scores (p<0,001) and from 40.71 ± 31.00 to 59.63 ± 27.61 scores respectively (p<0,05), viability increased from 30.59 ± 12.22 to 53.48 ± 12.50 scores (p<0,001), social health and the general health statuts improved from 22.02 ± 10.86 to 57.7 ± 9.84 scores (p<0,001) and from 20.03 ± 9.67 to 53.56 ± 8.94 scores respectively (p<0,001). It is concluded that both separate and combined application of f hypoxically hypercapnic stimulation and amplipulse therapy as a component of the spa and health resort-based treatment improves the quality of life of the patients suffering from bronchial asthma. The effectiveness of the treatment is especially high when the two therapeutic modalities are used in the combination.

The application of photoinactivation of Proopiobacterium acnes by laser radiation in the violet through red wavelength range for the treatment of acne

Abstract

The infection by Proopiobacterium acnes usually provokes the development of acne, i.e. the disease of sebaceous glands. The present study has demonstrated sensitivity of P. acnes to laser radiation in the violet (405 nm) through red (650 nm) wavelength range. The comparative in vitro investigation into the bactericidal properties of the violet and red radiation responsible for the eradication of P. acnes showed that radiation of the violet (405 nm) spectrum has a more pronounced influence on the pathogen than that of the red (650 nm) spectrum. The bactericidal effect is especially well apparent in case of the combined laser radiation in the violet through red wavelength range. Also, the in vitro experiments gave evidence that the action of laser violet radiation on P. acnes is 6 times stronger than that of red radiation; the former treatment strategy makes it possible to inactivate 95% of the colonies of this microorganism.

Photodynamic systemic biological therapy of the severe forms of psoriasis

Abstract

The present-day prevalence of psoriasis is characterized by the enhanced frequency of its torpid forms resistant to the treatment by the traditional methods and the ever growing occurrence of severe disease responsible for the significant deterioration of the patients’ quality of life and even incapacitation. The standard treatment of the patients presenting with severe forms of psoriasis resistant to the traditional therapeutic modalities includes the application of biological therapy. The use of the biological preparations produces, in the majority of cases, the beneficial effect. However, the development of the torpid process dictates the necessity of including cytostatic or immunosuppressive preparations in the therapeutic regimen. At the same time, this approach is undesirable in certain cases of the local pathological process because of an elevated risk of adverse side effects and the development of uncontrollable systemic immunosuppression. We report in this article the observations of three patients treated with ustekinumab, the biological preparation for systemic therapy. Isolated psoriatic patches torpid to the treatment persisted in these patients after the injection of ustekinumab on the 28th week. A session of photodynamic therapy (PDT) with the use of methyl aminolevulinic acid additionally prescribed to each patient resulted in positive dynamics of the cutaneous pathological process as apparent from the estimation of the values of the prevalence and severity indices of psoriasis, the size of the affected skin area, and characteristics of the patients’ quality of life. It is concluded that PTD may be recommended for the introduction in the system of treatment of psoriatic patients.

The evaluation of the effectiveness of the treatment of psoriasis and circumscribed scleroderma by means of pulsed laser therapy based on the results of high-frequency ultrasound scanning and the analysis of skin indices

Abstract

This paper was designed to report the data concerning the effectiveness of the method of pulsed laser therapy used to treat the patients presenting with moderately severe torpid psoriasis and local scleroderma. The results of the study give evidence of the high effectiveness of this therapeutic modality as apparent from the values of the dermatological indices and the results of high-frequency ultrasound scanning. Also, a high degree of correlation has been demonstrated between the values of the dermatological indices and the bacterial asymmetry coefficients of the skin parameters developed for the application together with the high-frequency ultrasound scanning technique.

The influence of EHF-therapy on the structure of bile in the patients presenting with chronic cholecystitis in combination with opisthorchiasis

Abstract

The importance of the development of the effective methods for the treatment of the patients presenting with chronic cholecystitis in combination with the opisthorchiasis arises from the fact that the functional disturbances in the biliferous system are responsible for destabilization of bile and the possible development of cholelithiasis. The timely beginning of the combined treatment including EHF-therapy promotes the improvement of indicators of the biochemical structure of bile with the normalization of its colloidal stability along with the simultaneous correction of the motor and evacuation function of the gallbladder and the enhancement of the adaptive resources of the organism. It ensures the high therapeutic effectiveness of the treatment without detriment to its tolerance.

The modern physiotherapeutic technologies for the rehabilitative treatment of the patients presenting with the disordered functions of the organ of vision

Abstract

The problems pertinent to the non-medicamental rehabilitative treatment of the patients presenting with the disordered functions of the organ of vision are discussed with special reference to the application of the modern physiotherapeutic technologies. It was shown that laser irradiation in the combination with transcutaneous electrical stimulation of the eyelids with the use of an Amplipulse-5D apparatus employed to treat myopia increases and stabilizes visual acuity which, in its turn, improves blood circulation in the retina, chorioidea, and the ciliary muscle of the eye.

The control of respiration modifies variability of the cardiac rhythm in the volunteers presenting with variations of the heliogeomagnetic factors

Abstract

The present work was designed to study the modification of the relationship between variability of the cardiac rhythm (VCR) and variations of the geomagnetic factors in 20 healthy female volunteers by means of regulation of their respiration with a frequency chosen on an individual basis during 50 days. It was shown that such regulation resulted in a change of the cardiac rhythm that was indicative of the enhanced adaptive potential of the organism and the reduction of tension in its regulatory systems in response to a given physical factor. Moreover, the regulation of respiration with the desired frequency modified the relationship between variability of the cardiac rhythms of the volunteers and the variations of the heliomagnetic factors as a result of reduction of the dependence of the vegetative regulation of the cardiac function on fluctuations of the heliomagnetic activity. The results of the present study may have practical implications for modern medicine and rehabilitation because they open up the prospects for the application of the method of the regulation of respiration with a frequency chosen on an individual basis for the correction of adverse changes in the functional state of the organism under the influence of fluctuations of the heliomagnetic activity. The data obtained give reason to recommend the courses of controlled respiration for the enhancement of the adaptive potential of the human organism.

The differential application of phototherapy for the treatment of psoriasis

Abstract

This review of the literature was designed to present the recent data on etiology and pathogenesis of psoriasis with special reference to the methods for the treatment of this pathology. Phototherapy is known to be of primary importance for the management of the patients presenting with this condition. Modern standards of the treatment of psoriasis envisage both the traditional ultraviolet therapeutic techniques and the innovative technologies with the use of laser and high-intensity radiation sources. Information concerning the effectiveness of the treatment of psoriasis in the children is presented.

Abstract

This article contains information concerning the complications that most frequently develop in the patients undergoing the surgical intervention for the treatment of breast cancer and the data on the possibility and desirability of the application of physical therapeutic methods for the rehabilitation of such patients belonging to the clinical group III. The methods for the performance of low-frequency low-intensity magnetic therapy, balneotherapy, therapeutic physical exercises, and massage are described for the purpose of elimination of postmastectomy lymphatic and lymphovenous oedema of the upper extremity on the affected side. The indications and contraindications for the prescription of therapeutic physical factors are discussed. The data on the major medications most frequently used for the rehabilitation of the patients with the help of the physiotherapeutic apparatuses are presented.
