Matvienko V.V., Tsoller K.A., Buchnov A.D.

Abstract

Intense mental activity of the students during academic studies involves mechanisms of adaptation and is associated with strong psychoemotional stress that affects their working ability and places specific demands on all body systems. The objective of the present work was to reveal and correct disturbances in the psychoemotonal sphere of the students of a medical college characterized by the low level of neuropsychic stability. The comprehensive study included 124 subjects (70 women and 54 men) of the mean age of 20 ± 1.2 years. The psychophysiological testing was carried out with the help of the “Multipsikhometr” programmed psychodiagnostic complex (PPK) used to estimate parameters of the psychic status, specific personality traits, working ability, and quality of life. The program for medico-psychological rehabilitation of the students characterized by the low level of neuropsychic stability (NPS) included the use of the methods for psychological correction of certain general disorders, such as breathing-relaxation training and autogenic training exercises. The following physical methods of rehabilitation were applied: the osteopathic techniques designed to normalize affected macro- and microlability of human tissues responsible for functional disturbances and compromised relaxation mechanisms. Therapeutic osteopathic procedures were carried out once weekly at the most, each lasting 40 minutes. The total duration of the treatment varied from 1.5 to 2 months. The clinical and physiological examination provided the data on the character and manifestation correction of the signs of neuropsychic instability in the students of the medical college. It was shown that correction of the psychomotional status in the subjects presenting with the low NPS level resulted in the normalization of the characteristics of the background psychoemotional conditions. Parameters of the quality of life of the students with the low NPS level were below normal ones when estimated based on the scales of both physical and psychological health conditions. The results of the study were employed to substantiate the use of parameters of the osteopathic status for the assessment of the effectiveness of the programs for the correction of psychoemotional state of the students with the low NPS level. The results of the factor analysis of individual characteristics of the functional state indicate that psychoemotional factors are responsible for 44% of the overall health status in the students presenting with the low level of neuropsychic stability.

The use of the natural and physical factors for the correction of metabolic processes in the patients presenting with metabolic syndrome

Frolkov V.K., Mikhailyuk O.V.

Abstract

The poor effectiveness of pharmacotherapy of metabolic syndrome dictates the necessity of development of a new method for the treatment of this disease; the most promising approach to achieving this goal appears to be the performance of special physical exercises. We undertook the study of the influence of physical loading with the use of a cardiovascular machine, pneumatic exercise equipment, and aqua-aerobic exercises in combination with the intake of mineral water. The outcome of the treatment was evaluated based on dynamics of carbohydrate and lipid metabolism, insulin and cortisol secretion, parameters of arterial pressure and body mass index. It was shown that physical exercises promoted regression of the manifestations of metabolic syndrome, the aqua-aerobic exercises being the most efficacious tool leading to the desired goal. The consumption of mineral water proved to enhance therapeutic effects of physical exercises. It was confirmed that the therapeutic effect of the combined treatment is realized via both the decrease in the activity of pathogenetic reactions associated with metabolic syndrome and the activation of sanogenic processes.

The application of electrical stimulation based on the ciliary reflex system for the combined treatment of the patients presenting with traumatic mandibular nerve neuropathy following dental implantation

Tatarkhanov N.V., Gerasimenko M.Y., Lazarenko N.N., Amkhadova M.A.

Abstract

The authors describe a new method for the rehabilitation of the patients in the early postoperative period following dental implantation in the mandible that was accompanied by the development of traumatic mandibular nerve neuropathy. All the patients were treated using standard therapy. Those in the study group underwent in addition electrical stimulation based on the ciliary reflex system with the application of bipolar pulsed currents generated in a Miomodel-10 device. The electrical stimulation based on the ciliary reflex system is designed to influence extracranially the peripheral component and the nuclear apparatus of the trigeminal nerve. The reflex arc of the ciliary reflex involves afferents of the trigeminal nerve, efferents of the facial nerve, nuclei of these cranial nerves, and neurons of reticular formation of the brain stem. Electrical stimulation based on the ciliary reflex system includes symmetric application of bipolar pulsed currents to the region of projection of the trigeminal nerve I, II, and III branches and Gasseri ganglion that gives rise to the first sensitive neurons whose peripheral dendrites form three major branches of the trigeminal nerve, viz. ophthalmic nerve (n. ophthalmicus), maxillary nerve (n. maxillaris), and mandibular nerve (n. mandibularis). In other words, electrical stimulation based on the ciliary reflex system acts extracranially over a large area,, eliminates pain syndrome by acting through neural conductors, improves blood circulation in these regions, and thereby facilitates the recovery of conductance through the nervous fibers of the mandibular nerve. These findings indicate that the application of bipolar pulsed currents by the local method and by electrical stimulation based on the ciliary reflex system promotes normalization of the functional activity of the mandibular nerve, activates microcirculation and neuroplastic processes, exerts analgesic, anti-inflammatory, regenerative, and neurotrophic actions.

Raigorodsky Y.M., Sharkov S.M., Urnyaeva S.I., Malykh A.L.

Abstract

The present study included 68 children of the mean age 9.6 years presenting with nocturnal enuresis treated by transcranial electrical stimulation (TES therapy) with the use of an AMO-ATOS-E apparatus. The clinical signs of imperative urination were evaluated based on the E.L. Vishnevsky questionnaire. The urological investigations included uroflowmetry (UF). All the patients underwent EEG examination before and after treatment. The patients of the control group were treated by basal therapy with the use of driptane (oxybutinin). Electrical stimulation was applied to the frontomastoid region with the commutation of the right and left “mastoid“ electrodes at a frequency of 10 Hz. The treatment course consisted of 8-10 procedures repeated 2-3 times every week. TES therapy resulted in the elimination of the symptoms of enuresis in 73.6% of the patients (compared with 50% in the control group). The mean effective bladder volume in the children of the study group increased by 63.4% compared with 38.8% in controls. The results of analysis of EEG suggest that the frequency of registration of the alpha-rhythms in the study and control groups increased by 23.7% and 6.2% respectively.

Rehabilitation of the children having the history of exposure to the unfavourable ecological conditions based at spa-and-health resort faclities

Shvedunova L.N., Glukhov A.N.

Abstract

The study included 350 children at the age varying from 7 to 14 years who arrived from the regions with unfavourable ecological conditions. All of them were found to have functional abnormalities in various organs of the psychoemotional sphere, nervous. endocrine, immune, cardiovascular, and digestive systems apparent as the manifestations of ecological disadaptation syndrome. It was shown for the first time that rehabilitation of such patients is possible by drinking mineral water in the framework of comprehensive spa-and-resort-based therapy. Balneotherapeutic procedures prescribed to the children having the history of exposure to the unfavourable ecological conditions contributed to the optimization of metabolic reactions mediated through switching catabolic processes to anabolic ones as confirmed by the increment of in the body weight and the two-fold increase of the insulin/cortisol coefficient. The study demonstrated that the consumption of double doses of Slavyanovskaya mineral water produced especially well apparent therapeutic effect due to improved dynamics of asthenic signs; moreover, combination of its standard doses with a pectin extract is especially advantageous for the alleviation of severe pain and dyspeptic syndromes. Simultaneously, this therapeutic modality had beneficial effect by reducing the elevated levels of many hormones and caused a 20% decrease in the excessive triiodothyronine activity. A similar therapeutic effect was documented in the children that consumed traditional doses of Essentuki N4 mineral water.

Rehabilitation of the children after the surgical treatment for juvenile cerebral palsy based at a Pyatigorsk health resort

Krivobokov V.N., Cherevashchenko L.A.

Abstract

The importance of the problem of juvenile cerebral palsy (JCP) is due to the enhanced prevalence and social significance of this condition leading to severe disabilities. The rehabilitative treatment of these patients should be initiated as early as possible in the form of combined and intense therapy. Of special value in this context is the application of natural therapeutic factors in adequate doses to enhance the reserve potential of the functional system and facilitate the development of adaptive reactions. The spa-and-resort-based treatment is considered to be the most physiologically sound and efficacious approach to the rehabilitation of the children presenting with juvenile cerebral palsy. The authors of the present publication undertook the development of a scientifically sound system of the health resort-based rehabilitative treatment of the children who had undergone surgical intervention for the management of JCP in the preceding period. The previously gained experience with the use of differential therapeutic modalities gives evidence that it is desirable to include the spa-and-resort-based treatment in the programs of rehabilitation for the children following surgical treatment of juvenile cerebral palsy. The results of direct observations are confirmed by the data of follow-up studies suggesting that the beneficial effects of the medical technologies employed in the present work persist for at least half a year.

The application of electroconvulsive therapy and plasmapheresis for the management of refractory conditions in psychiatry

Donenko V.E., Kuznetsov A.V., Raigorodsky Y.M.

Abstract

The present study included 36 patients with the diagnosis of schizophrenia and affective disorders refractory to pharmacotherapy. The study has demonstrated the effectiveness of electroconvulsive therapy with the use of an ESTER apparatus (ECT) and plasmapheresis (PP). ECT therapy was conducted using a 30 second electric charge at a pulse frequency of 27 Hz and duration of 0.5-1.5 ms. Under these conditions, the dose of ECT therapy was 150 mKl. Isolated electroconvulsive therapy and its combination with plasmapheresis decreased pathological manifestations of the disease in 29.4% and 42.1% of the patients respectively. Partial reduction in the former case was estimated at 52.9% compared with 52.6% in the latter case. Complications in the form of elevated arterial pressure, nausea, and vomiting after awakening, mnestic disorders, and tachycardia in the former and the latter cases were documented in 17.6% and 5.2% of the treated patients respectively.

The development of up-to-date health promotion programs for the enhancement of the functional reserves of the body in the students engaged in active sports activities

Ivanova I.I., Kotenko K.V., Korchazhkina N.B.

Abstract

The present study was designed to evaluate the possibility of health protection in the students engaged in active sports activities laying emphasis on the application of the non-medicamental technologies for the optimization and enhancement of the functional reserves of physical health. The study involved 320 students at the age from 18 to 21 years. At the first stage, the students engaged and unengaged in active sports activities were compared in terms of their functional conditions with the control healthy subjects. At the second stage, the students involved in active sports activities were included in the complex health promotion program to undergo reflex-segmental, vacuum-interference, and general treatment, such as combined application of a pulsed low-frequency electrostatic field, baths containing biologically active compounds of plant origin (components of horse chestnut) and monotherapy. Comparative analysis of the results of these investigations has demonstrated the advantages of the complex health promotion program confirmed by normalization of the vitality index, Robinson’s index, and Martine-Kushelevsky test as well as integral index dynamics to the respective reference values. The results of the study give evidence that the methods of general treatment cause a more apparent increase of the studied parameters than reflexo-segmental ones.
