Diagnostics and the combined medical tactics at patients with erectile dysfunction at a empty sella syndrome

Abstract

Background. The study of hormonal status in patients with erectile dysfunction (ED), especially the older and senior age groups, is most often limited to the evaluation of testosterone fractions

Aim: demonstrate the effectiveness of transcranial magnetotherapy in a combined treatment regimen for patients with «empty sella syndrome” (ESS).

Methods. Results of diagnostics examination and treatment of 118 patients with the ED hormonal form at the age of 23–57 are studied. The expressiveness of disturbances of erectile function and signs of androgenic deficiency are estimated by means of questionnaires of AMS and a scale of Morley, the penil color flow Doppler ultrasound with intracavernous pharmacological tests is carried out, the hormonal status is estimated. At suspicion of neuroendocrine pathology MRI of the brain is executed. At all examined patients signs of decrease erectile functions are revealed. In 62 (52.5%) observations during MRI the picture of ESS is revealed. In 22 (18.6%) observations the changes of an eyeground demonstrating increase in intracranial pressure takes place, however loss of fields of vision is not observed in any of patient. Persistent headaches are noted in 19 (16.1%) observations. In 46 (39%) observations at patients with considerable obesity signs of disturbance of tolerance to glucose and insulin resistance are revealed. To patients hormone replacement therapy is appointed, at identification of a hyperprolactinemia agonists of dopamine receptors in an individual dosage under control of level of prolactin are used, therapy of intracranial hypertensia is carried out. In complex therapy transcranial magnetotherapy is used.

Results. Therapy is effective, during treatment increase in frequency and duration of sexual intercourses is noted. At control Doppler researches of a penis improvement of a regional hemodynamics is noted. Cancellation of medicines lead to resuming of symptomatology of a disease.

Conclusion. Thus, the research shows powerful value of a syndrome of ESS in a pathogeny of ED demanding multidisciplinary approach with involvement of adjacent specialists — endocrinologists, neurologists and ophthalmologists. It is important to note that ESS, in most cases, is an accidental tomographic finding, at the same time often is followed by disturbance of tolerance to glucose, obesity, a metabolic syndrome. Introduction in complex therapy of transcranial magnetic influence substantially improves results of treatment of this category of patients, as due to additional stimulation of synthesis of pituitary hormones, and cerebral decompression.

Assesment of osteoporotic fractures risk and osteoporosis prevalence among patients over 50 years old undergoing medical rehabilitation

Abstract

Background. Back pain, limitation of mobility, spinal deformity, decreased ability to self-care due to osteoporotic fractures (vertebrae, femoral neck, humerus, etc.) are one of the reasons for referring patients to rehabilitation and rehabilitation treatment, which involves the use of physical therapy methods, physiotherapy exercises and mechanotherapy.

Objective: to assess the prevalence of osteoporosis (OP), individual risk factors for OP, as well as the proportion of people with high risk of osteoporotic low-energy fractures among patients over 50 years old undergoing treatment according to the “medical rehabilitation” profile.

Methods. The study group comprised of 600 patients (426 women and 174 men) aged 50 to 84 years, average age 64.25±10.17 years, undergoing treatment in a rehabilitation department. This was a cross-sectional study in the form of unified questionnaire, including data conserning age, weight, height, BMI, clinical and rehabilitation diagnosis, anamnesis of the main disease, anamnesis vitae, presence of OP diagnosis in the anamnesis, its treatment, OP risk factors estimation. An assessment of 10-year probability of osteoporotic fractures was carried out using Russian model of online FRAX® calculator.

Results. 41.8% patients in the study sample had OP risk factors, including 31.2% ― 3 risk factors or more. 38% patients over 50 years old undergoing medical rehabilitation showed a high fracture risk according to the FRAX calculator. 34.1% had a diagnosis of OP stated, and 45.8% already had osteoporotic fractures. Among those who did not undergo densitometry examination, 69.9% had a history of low-traumatic fractures, and only 58.5% of patients with an established diagnosis of OP and 26.8% of those at high risk of fractures received effective therapy for OP.

Conclusion. Among patients over 50 years old undergoing medical rehabilitation a high frequency of OP and high fracture risk were stated. At the same time, insufficient quality of OP diagnostics and anti-osteoporotic therapy administaration were revealed.

Medical rehabilitation of children with hydronephrosis with concomitant neurogenic bladder dysfunction

Abstract

Background. After operations on the ureter, it is important to use medical rehabilitation technologies aimed at preventing urodynamic disorders of the upper and lower urinary tract, stimulating reparative processes. In recent years, special attention has been drawn to high-intensity pulsed magnetotherapy, which has a more pronounced and long-lasting stimulating effect compared to other types of electrical stimulation.

Aim: to substantiate the feasibility of including high-intensity pulsed magnetic therapy in the comprehensive medical rehabilitation of children with hydronephrosis and concomitant neurogenic bladder dysfunction.

Methods. Clinical observations and special studies with the inclusion of high-intensity pulse magnetic therapy in the course of therapy were conducted in 100 children aged 4 to 15 years with hydronephrosis, a condition after surgery.

Results. Medical rehabilitation with the inclusion of high-intensity pulsed magnetic therapy had a positive effect on the urodynamics of the urinary tract, renal blood flow, helped to stop inflammation in the renal parenchyma and restore the rhythm of urination in children with hydronephrosis and concomitant neurogenic bladder dysfunction.

Conclusion. According to the research results, the feasibility of including high-intensity pulsed magnetic therapy in the complex of medical rehabilitation of children with hydronephrosis, concomitant neurogenic bladder dysfunction, is scientifically justified.

Combined physical therapy for diabetic angiopathy

Abstract

Background. The problem of diabetic angiopathy is characteristic of countries even with a high level of development of medicine. The high frequency of complications of diabetes with high biological and social significance determines the need for their correction.

The purpose of the work was to increase the effectiveness of rehabilitation treatment for patients with diabetic angiopathy through the combined use of therapeutic exercises and functional electrical stimulation of the lower extremities.

Methods. A prospective cohort study was conducted, in which 90 patients (63 men and 27 women) with an established diagnosis of type 2 diabetes took part.

Results. It was found that the use of therapeutic gymnastics and training walking on a treadmill and functional neuroelectrostimulation of the lower extremities increase the motor capabilities of patients due to the analgesic effect, improved neurogenic control of vasoconstriction of arteriovenous anastomoses and increased capillary blood flow.

Conclusion. Functional electrical stimulation method can be effectively used in physical therapy programs for patients with diabetic lower limb angiopathies at the inpatient and outpatient stages.

The effectiveness of the combined use of fractional microneedle rf-therapy and pharmacophoresis of hyaluronic acid in the treatment of patients with atrophic scars

Abstract

Background. The formation of scar tissue is a fairly physiological process, in most cases aimed at closing the skin defect in order to preserve the body’s homeostasis. Correction methods for atrophic scars include laser technology, radiofrequency therapy, injection methods and topical therapy.

Methods. There were 56 patients with atrophic scar. Depending on the therapeutic complex for the correction of аtrophic scar, 3 groups were formed: group 1 ― the combined use of RF-therapy for the pharmacophoresis of hyaluronic acid, group 2 ― radio frequency microneedle therapy, group 3 ― the pharmacophoresis of hyaluronic acid. In the work, the Dermatological Symptom Scale Index (DSSI), which was assessed before and after treatment, was adapted to evaluate clinical symptoms.

Results. In accordance with the total value of the DSSI, a more significant dynamics was observed in group 1, regardless of the type of AR (subgroups 1A and 1B). DSSI in the 1A subgroup decreased by 80.8% (p<0.01), in the 1B group by 82.4% (p<0.01). DSSI in the 2A subgroup decreased by 32.3% (p<0.01), in the 2B group by 61.4% (p<0.01). DSSI in the 3A subgroup decreased by 62.3% (p<0.01), in the 3B group by 33.7% (p<0.01). In group 1, where the combined treatment was carried out, a more pronounced efficacy was noted than when applying the components of the technique, which did not depend on the type of cicatricial deformity. In group 2, after the application of RF-therapy, the effectiveness was expressed to a much lesser extent, and the most significant results were obtained in patients with post-traumatic atrophic scars. In group 3, after a course of pharmacophoresis of a hyaluronic acid preparation, positive dynamics was observed to a greater extent with hormone-dependent scars.

Conclusions. The results of the study showed that the most effective method is the combined use of RF-therapy and the pharmacology of hyaluronic acid, at the same time with post-inflammatory (post-traumatic) atrophic scars, monotherapy with the use of radiofrequency microneedles is possible, with hormone-dependent atrophic scars, monotherapy of pharmacophoresis of hyaluronic acid is possible.

Experimental study of cooling spray for physiotherapeutic treatment

Abstract

For cooling damaged areas of biotissue in order to achieve a rapid analgesic effect, a procedure for applying a thin film to the skin, boiling at a temperature of T0 = 273…238 K of a gas mixture, is widely used. This temperature is achieved when using a cooling spray. Experimental data on the temperature distribution on the cooled surface when throttling a propane/butane/R123 mixture from a nozzle with a diameter of 0.5 mm to the temperature level Т0 = 240; 263; 270 К, used for physiotherapy purposes, are presented. A comparison of the efficiency of cooling the skin using a cryopresponder and using ice was also made. It is shown that cooling with a cooling spray is more efficient.

Effectiveness of pharmacopuncture by “alloplant” biomaterial in complex therapy of knee joint osteoarthritis

Abstract

Background. Osteoarthritis is characterized by a decrease in the quality of life, which is currently considered as the main measure of well-being of an individual and a criterion for the effectiveness of treatment and rehabilitation measures. Modern research emphasizes the need for an integrated approach to the treatment of osteoarthritis, which should include pharmacological and non-drug methods of treatment.

Aim. The purpose of the study was to evaluate the effectiveness of pharmacopuncture administration of Alloplant biomaterial by patients with osteoarthritis of the knee joint from the standpoint of influencing the immunological mechanisms of the disease.

Methods. The randomization methodology was divided into 3 groups, comparable in terms of clinical and functional characteristics. Group I patients (n = 39) have standard basic drug therapy according to the federal clinical guidelines “Osteoarthritis” approved by the Association of Rheumatologists of Russia (2013), with additions from 2016; in patients of group II (n = 39), in addition to drug therapy, dispersed Alloplant biomaterial was used according to the original method; group III patients (n = 39) against the background of basic drug therapy are additionally prescribed a combination of dispersed Alloplant biomaterial at biologically active points and magnetic therapy with a pulsed magnetic field.

Results. Against the background of the treatment of patients with osteoarthritis using pharmacopuncture with Alloplant biomaterial and magnetotherapy, there was a positive dynamics in clinical data with a significant decrease in pain intensity when walking and at rest, stiffness, with the restoration of immune status parameters and an improvement in the quality of life in the field of physical and psychological health . A more pronounced and lasting positive dynamics was observed in patients of group III against the background of the combined effects of pharmacopuncture with Alloplant biomaterial and magnetotherapy.

Conclusion. The Alloplant biomaterial has a positive effect in the treatment of patients with stage I–II osteoarthritis according to the Kellgren–Lawrens classification. The similarity of the mechanism of action with drugs with a structurally modifying effect on cartilage allows us to recommend pharmacopuncture with Alloplant biomaterial for widespread use in the treatment of osteoarthritis.

The effectiveness of a new complex of physical rehabilitation with the inclusion of mechanotherapy technologies in improving quality of life in patients with osteoporotic vertebral fractures

Abstract

Background. Medical rehabilitation plays a key role in restoring functionality and social activity, as well as the physical and psychological aspects of quality of life (QOL) in patients with pathological fractures in the setting of osteoporosis (OP).

Objective: to assess the effect of a new complex of physical rehabilitation with the inclusion of mechanotherapy on the physical and psychological aspects of QOL in patients with vertebral fractures (VFs) and OP.

Methods. The study was carried out in the form of a prospective controlled open study in two parallel groups. 90 patients with osteoporotic VF were randomized in a 2:1 ratio to the intervention group (group 1, n=60), which received a new complex of physical therapy, or control group (group 2, n=30), which received only special complex of physical exercises. QOL and pain level assessment was performed at baseline and at follow-up on the 21st and 70th day in patients of both groups. A special questionnaire QUALEFFO-41 was used to assess QOL.

Results. Administration of a new physical rehabilitation complex resulted in pain reduction and improvement of such QOL aspects as jobs around the house, mobility and mental state. No dynamics was found in QUALEFFO-41 scale in “daily living activity” and “leisure, social activities”, “general health perception” domains. Therapy effect on pain syndrome relief, expansion of daily living activity, mobility and increasement in overall QOL assessment, remains for at least 4 weeks after the rehabilitation course.

Conclusion. New three-week physical therapy complex, including mechanotherapeutic methods with biological feedback and special physical exercises in the gym and pool can be recommended for the rehabilitation of patients with osteoporotic VF in order to increase QOL and reduce back pain.

Some results of studying of mechanisms of effect of drinking mineral waters and prospect of further development of this problem

Abstract

In article the main achievements in a problem of studying of mechanisms of treatment-and-prophylactic use of drinking mineral waters are analysed. The important role of their nonspecific influence due to existence of a stressorny component, activation of digestive organs and increase in products of gastrointestpnalny and pancreatic hormones is proved. Increase in activity of enteroinsulyarny interrelations at intake of mineral waters provides optimization of the broken metabolic reactions. It is revealed that against the background of course intake of mineral waters the resistance to insulin due to optimization of hormonal and receptor interaction decreases. The role of activization of pro-oxidatic reactions in improvement of sensitivity of fabrics to insulin is discussed. Various methods of increase in medical potential of mineral waters due to change of algorithms of their use, enrichment are analyzed by vitamins and phytodrugs, nanoparticles of metals. The scheme of realization of biological potential of mineral water at its internal reception is offered.

The effectiveness of electrophoresis with caripain in the treatment of discogenic radiculopathy in the cervical spine. Clinical case.

Abstract

In this article, we demonstrate a clinical case with high efficacy of Karipain electrophoresis in the treatment of a patient with discogenic radiculopathy in the cervical spine. The positive result was a complete regression of motor deficiency, restoration of sensory function and a decrease in the size of a hernia of the intervertebral disc at the level of C5–C6 by 40%.
