Dynamics of clinical and functional indicators in patients with the consequences of combined facial skin trauma during the implementation of comprehensive rehabilitation programs

Abstract

 
 

Rationale: Combined facial skin trauma (CFST) often leads to scars and traumatic tattooing, causing substantial cosmetic and psychological distress. Effective evidence-based rehabilitation strategies are lacking due to a scarcity of controlled studies.

Objective: This study aimed to assess changes in clinical, functional, psychophysiological, and quality of life (QoL) outcomes in patients with CFST sequelae following comprehensive rehabilitation.

Materials and Methods: A prospective randomized comparative trial was conducted involving patients with CFST sequelae. Participants were randomized into two groups: the study group received combined rehabilitation using laser technologies and carboxytherapy, while the control group received standard care (facial massage, phonophoresis with lidase, and methyluracil ointment).

Primary endpoints included scar and pigment severity scores. Secondary endpoints comprised instrumental skin measurements, psychophysiological parameters, and QoL assessments. The rehabilitation program duration was 4 months per patient.

Results:
All 71 enrolled patients completed the study without adverse events.

The study group demonstrated significantly greater improvement in rehabilitation effectiveness (p<0.001) compared to the control group.

Tissue stiffness was significantly lower in the study group (median=25.3 kPa) than in the control group (median=30.9 kPa; p<0.05). The intervention group also showed significantly better outcomes in skin texture, surface morphometry, pigment levels, hemoglobin concentration, and lymph flow.

Psychophysiological parameters improved significantly only in the study group. QoL assessments using the WHOQOL-100 questionnaire confirmed superior outcomes in the intervention group.

Conclusion: The comprehensive rehabilitation approach incorporating high-intensity laser therapy and carboxytherapy proves significantly more effective than standard therapy for managing CFST sequelae, improving both physiological and psychological outcomes.
