The role of the thermal component in the therapeutic action of physical factors

Abstract

It has been shown in this work that the thermal component of physical and balneological factors in a certain dose range plays an extremely important role in the manifestations of their therapeutic effects. The initial stage of thermoregulatory processes is mediated through thermoreceptors the action of which is realized via triggering adaptive mechanisms of the body at the local and systemic levels. These mechanisms ensure the generalized response to the thermal stimuli. Individual components of this response include stimulation of blood supply of the tissues, their metabolism and regeneration in conjunction with anti-inflammatory, analgesic, and immunoregulatory effects. These effects are attainable on condition of the correct choice of the therapeutic regime and the site of application of the thermal factors taking into consideration contraindications, if any, in each concrete case. It is concluded that infrared and visible radiation from laser and non-laser sources affect hemodynamics in the treated regions by virtue of photochemical stimulation of the synthesis of nitric oxide, a mediator of hemodynamic shifts.

Peculiarities of rehabilitation of the patients presenting with cardioembolic stroke with concomitant chronic cardiac insufficiency

Abstract

The present work was designed to estimate the safety and efficacy of the application of a combination of rehabilitative modalities during the acute period of cardioembolic stroke (CES) in 28 patients displaying different degree of decompensation of the cardiac function. The study has demonstrated that the use of "passive" rehabilitative measures, such as postural therapy, respiratory gymnastics, selective massage, and neuromuscular electrostimulation, is safe for the patients who experienced cardioembolic stroke and suffer decompensation of the cardiac function (functional class II and III chronic heart failure). It is emphasized that the introduction of active therapeutic modalities into the programs of combined rehabilitative treatment (e.g. verticalization, active remedial gymnastics, and subsequent walking training) may further worsen decompensated cardiac function in 53.5% of the patients with class III chronic heart failure; in such a case, an immediate correction of the prescribed treatment is needed. However, the majority of the patients benefit from the proposed regimen showing the improved working capacity, tolerance of physical exercises, and motor function, reduced severity of the stroke, and a higher degree of functional independence.

Dynamics of metabolic parameters in the patients presenting with arterial hypertension under the influence of general air-cryothermal factors

Abstract

The authors present the results of a clinical study aimed to estimate the influence of general air- cryothermal factors (with the temperature of the cooling medium in the range from -60 oC to -110 oC) on the functional status and dynamics of metabolic parameters in the patients presenting with mild arterial hypertension (AH). The study has demonstrated that the use of cryotherapy for the treatment of patients with AH accelerates the reduction of subjective clinical symptoms of the disease and optimizes the regulation of the function of the cardiovascular system. A course of general air-cryotherapy induces long-term adaptive shifts resulting in the improved regulation of vegetative functions and metabolic processes; moreover, it augments functional reserves of the organism, enhances non-specific resistance, improves mental and physical health of the patients. The inclusion of general air-cryotherapy in the combined treatment is in the first place indicated to the patients presenting with stage I-II AH and preserved functional potential because it causes marked mobilization of their physiological functions. It is recommended that general air-cryotherapy as described in the present publication should be carried out in conjunction with basic medicamental therapy under thorough control of the patient's clinical condition.

General magnetotherapy in the children with cerebral ischemia during the first months of life

Abstract

The literature data concerning pathogenesis of cerebral ischemia in the children are presented. Based on these data, the authors provide a rationale for the use of non-medicamental therapeutic modalities for the management of pathology of interest including general magnetotherapy following a specially developed protocol. The treatment of 152 patients with moderately severe cerebral ischemia during the first months of life made it possible to achieve positive dynamics of both hyporesorptive and liquor-dynamic cerebral processes.

The application of an electrostatic field for the combined treatment of remitting multiple sclerosis

Abstract

The present article is devoted to the application of a pulsed electrostatic field for the treatment of the children at the age from 14 to 17 years with remitting multiple sclerosis (residual neurologic deficiency including changes of sensation, coordination, and motor function) and invalidization estimated at 1.5-3.5 scores based on the expanded disability status scale, EDSS). All the patients given complex therapy with the application of a pulsed electrostatic field (PEF) positively responded to the treatment that reduced severity of neurologic deficiency, improved vegetative regulation and functioning of the peripheral neuromotor apparatus. The introduction of training coordination of the movements into the combined treatment based on the biological feedback mechanism proved a highly efficacious method for the correction of coordination disorders in the children with multiple sclerosis.

The authors present the results of clinical and functional studies that provide a rationale for the inclusion of general biolong bath therapy in the program of combined treatment of the patients presenting with chronic obstructive pulmonary disease and bronchial asthma. In addition, the studies demonstrate the advantages of the application of a balneological factor with the properties of an antihypoxant and antioxidant for the management of bronchial asthma.

Pathological locomotor synergies after stroke and their correction under the influence of training with the use of the Lokomat robotic system

Abstract

Acute disturbances of cerebral blood circulation are known to be a major cause of the long-standing loss of working capacity and the disablement in the affected patients. Over 50% of the subjects who survived acute disturbances of cerebral blood circulation suffer locomotor dysfunction of different severity. Both acute and subacute periods of cerebral stroke resulting in locomotor dysfunction are characterized by rapid development of pathological synergies known as the Wernicke-Mann posture and marked deterioration of gait quality. The objective of the present study was to evaluate the effect of a course of gait training with the use of the robotic Lokomat system on the severity of pathological synergies in 37 patients during the post-stroke period. Clinical and biomechanical parameters of the walking gait were estimated based on the formal scales and systems for videoanalysis of the movements. It was shown that walking training with the use of the Lokomat system makes it possible to reduce the severity of pathological locomotor synergies and improve functional mobility of the patients.

Abstract

The objective of the present study was to compare the results of the treatment of 349 patients presenting with severe acute pancreatitis complicated by the presence of pancreatogenic infiltrate. Combined therapy of all the patients included hyperbaric oxygenation (HBO) procedures and the application of ultrahigh-frequency electromagnetic radiation (UHF therapy). Analysis of the results of the study indicate that a combination of HBO and UHF therapy starting from the early period of the formation of pancreatogenic infiltrate promotes activation of non-specific adaptive reactions and decreases the frequency of development of pyo-destructive complications by a factor of two. At the same time, the use of HBO results in a significant increase of the number of patients with uncomplicated resorption of pancreatogenic infiltrate. This and other therapeutic effects of HBO indicate that this method has certain advantages in comparison with UHF therapy.

Endogenous intoxication as a manifestation of dysadaptation in high-class athletes

Abstract

The objective of the present study was to estimate the informative value of various methods of laboratory diagnostics of endogenous intoxication (EI) developing as a result of physical loads experienced by the athletes. It included 34 high-class athletes (17 men and 17 women), members of the national academic rowing team (holders of candidate and maser of sports). Physical loads in different periods of the annual training cycle (including basic training, precompetitive, competitive, and recovery stages) were compared with the blood levels of middle molecular weight proteins (MMWP) (biochemical markers of endogenous intoxication) in the respective time intervals. The comparison revealed a strong stress-related impact on the functional capacity of the organs responsible for natural detoxication under effect of enhanced anaerobic loading and special muscle-strengthening and speed exercises . It is concluded that determination of the plasma level of middle molecular weight molecules has the highest informative value among other methods for the estimation of the severity of endogenous intoxication based on biochemical characteristics. Weak and variable expression of cytokines in blood samples taken during competitive and recovery period of the annual training cycle in conjunction with the absence of appreciable dynamics of their plasma levels makes impossible their use as biochemical markers of endogenous intoxication in professional athletes.

Abstract

The authors analyse the literature publications concerning the methods of instrumental physiotherapy and spa-and-resort treatment of elderly patients presenting with arterial hypertension. It is concluded that therapeutic physical factors provide an important tool for the improvement of efficacy of the management of arterial hypertension in the patients of advanced age. This inference suggests the necessity and expediency of investigations into the mechanisms of their action, substantiation of medical indications and contraindications for their application, and the development of methods for the realization of the relevant therapeutic procedures in order to enhance the efficiency of the use of variety of the natural and preformed physical factors in geriatric cardiology.

Abstract

The approaches to the substantiation of physiotherapy for the patients presenting with acute disturbances of cerebral blood circulation (ADCBC) are considered with special reference to the symptomatic treatment for the alleviation of pain syndrome and normalization of cerebral microcirculation, activation of the reparative processes, maintenance of functional activity of the neuromuscular apparatus of the extremities, and prevention of the development of contractures. The major physiotherapeutic procedures to be performed within a period from 3 days to 1.5 months after the onset of ADCBC are described.
