However, post-traumatic and postoperative complications occur in a significant proportion of patients, ranging from 18% to 85% according to various reports. Of particular note is that persistent post-traumatic stiffness develops in 18% of affected individuals. Currently, the clinical aspects of mechanotherapy use in elbow motor function recovery in patients with post-traumatic contractures remain insufficiently investigated.</p> <p><bold>AIM:</bold><italic> </italic>To evaluate the efficacy of passive mechanotherapy using the Flex F03 device in elbow injuries complicated by contracture.</p> <p><bold>METHODS:</bold><italic> </italic>The study included 79 patients aged 18 to 72 years with elbow injuries and contractures who provided informed consent to participate in the study and to have their medical data published for scientific purposes. Patients were enrolled no earlier than the second day and no later than six months after immobilization was removed. The patients were randomized into two groups: the control group (36 patients) and the study group (43 patients). The diagnostic protocol included: clinical examination; pain and functional assessment using the visual analog scale (VAS); upper limb functional assessment using the DASH questionnaire; patient condition assessment using the Modified Rankin Scale; and motor deficit assessment using the MRC scale. Both groups received a standardized 10-day course of treatment, including therapeutic exercise, cervical massage, polychromatic polarized phototherapy, and magnetotherapy. In addition, patients in the study group underwent passive mechanotherapy using the Flex F03 device according to the authors’ method.</p> <p><bold>RESULTS:</bold><italic> </italic>After rehabilitation, both groups showed statistically significant improvements in the following parameters: increased flexion and supination angles in the elbow joint, reduced motor deficit in flexor and extensor muscles across all upper limb segments, decreased pain intensity, and improved daily activity (DASH). Significantly greater improvements were observed in the study group compared with the control group.</p> <p><bold>CONCLUSION:</bold> Passive mechanotherapy in the proposed regimen produces the following clinical effects: increased range of motion in the elbow joint, reduced upper limb motor deficit, decreased pain intensity, and improved daily activity. Early introduction of passive mechanotherapy into the rehabilitation process can shorten the duration of the rehabilitation course, reduce the number of required treatment courses, and increase the likelihood of achieving functional range of motion in the elbow joint.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold><bold> </bold>Травматические повреждения костей локтевого сустава относятся к относительно редким клиническим случаям, составляя не более 5% от общего числа скелетных травм. В то же время посттравматические и послеоперационные осложнения регистрируются у существенной доли пациентов — по разным источникам, в 18–85% случаев. Особого внимания заслуживает тот факт, что у 18% пострадавших формируется стойкая посттравматическая тугоподвижность локтевого сустава. На текущий момент недостаточно изучены клинические аспекты применения механотерапии в восстановлении двигательной функции локтевого сустава у пациентов с посттравматическими контрактурами.</p> <p><bold>Цель исследования.</bold><bold> </bold>Оценить эффективность применения метода пассивной механотерапии от аппарата «Flex-F03» при травмах локтевого сустава, сопровождающихся контрактурой.</p> <p><bold>Методы.</bold><bold> </bold>В исследование включены 79 пациентов в возрасте от 18 до 72 лет с повреждениями локтевого сустава, сопровождающимися его контрактурой, при наличии информированного согласия на проведение протокола исследования и опубликование медицинских данных в научных целях. В исследование включали пациентов не ранее второго дня и не позднее шести месяцев после устранения иммобилизации, обусловленной травмой или оперативным вмешательством. Методом рандомизации сформированы две клинические группы: группа сравнения (36 человек) и основная группа (43 человека). Диагностический протокол включал клиническое обследование, оценку альгофункционального индекса по визуально-аналоговой шкале, анализ функциональных нарушений верхней конечности по опроснику DASH, оценку состояния пациента по модифицированной шкале Рэнкина и определение двигательного дефицита по шкале MRC. Обе группы получали стандартизированный десятидневный курс терапии, включающий лечебную гимнастику, массаж воротниковой зоны, светолечение полихроматическим поляризованным некогерентным излучением, магнитотерапию. Пациентам основной группы дополнительно была применена пассивная механотерапия от аппарата «Flex-F03» по авторской методике.</p> <p><bold>Результаты.</bold><bold> </bold>По итогам реабилитационных мероприятий зафиксировано статистически значимое улучшение у пациентов обеих групп следующих показателей: увеличение углов сгибания и супинации в локтевом суставе, снижение двигательного дефицита мышц-сгибателей и разгибателей во всех сегментах верхней конечности, уменьшение интенсивности болевого синдрома и повышение уровня повседневной активности (DASH). В основной группе наблюдали статистически значимо более выраженные позитивные изменения относительно группы сравнения.</p> <p><bold>Заключение.</bold><bold> </bold>Применение пассивной механотерапии в предложенном режиме демонстрирует следующие клинические эффекты: увеличение объёма движений в локтевом суставе, уменьшение двигательного дефицита мышц верхней конечности, снижение интенсивности болевого синдрома, повышение уровня повседневной активности.
