Promising solutions include multimodal and safe methods of restorative medicine. We previously demonstrated the effectiveness of combined physiotherapy in reducing biological age; however, analysis of long-term outcomes is required for personalization of preventive strategies.</p> <p><bold>AIM:</bold> This study aimed to evaluate changes in patients’ condition in the long-term period after comprehensive physiotherapy and to statistically assess predictors of active longevity prolongation based on these findings.</p> <p><bold>METHODS:</bold> This was a clinical, prospective, randomized, controlled, comparative study involving 80 patients aged 20 to 90 years. Using simple fixed randomization, all patients were divided into two groups: a control group and a main group. Patients in the control group received moderate physical activity, caloric restriction, and vitamin therapy. Patients in the main group additionally received a physiotherapeutic complex that included multimodal interventions using the Alpha™ LED Oxу Light Spa™ device, hypoxic–hyperoxic therapy, pressotherapy, and dry carbon dioxide baths. Preventive courses were repeated twice at 6-month intervals. All assessments were performed before each preventive course and 6, 12, and 18 months after its completion. Multiple linear regression analysis and correlation adaptometry were applied to identify predictors.</p> <p><bold>RESULTS: </bold>Comprehensive physiotherapy was found to reduce biological age and the rate of aging not only immediately after the course but also for up to one year. The effect was associated with a reduction in insulin resistance, atherogenicity, and inflammation, along with a slight increase in telomere length. Predictors of long-term effect included low levels of insulin resistance, inflammation, and oxidative stress in combination with increased telomere length.</p> <p><bold>CONCLUSION: </bold>Premature aging is associated with systemic disturbances, primarily in insulin regulation of carbohydrate and lipid metabolism. The use of restorative medicine methods (device-based physiotherapy and natural therapeutic factors) is highly effective, as they safely stimulate hormonal regulation of metabolism, are accessible and easy to use.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Разработка программ активного долголетия затруднена из-за сложности исследований и коморбидности пациентов. Перспективным решением являются полимодальные и безопасные методы восстановительной медицины. Ранее мы доказали эффективность комбинированной физиотерапии в снижении биологического возраста, однако для персонификации профилактики необходим анализ отдалённых результатов.</p> <p><bold>Цель.</bold><italic> </italic>Изучение динамики состояния пациентов в отдалённом периоде после применения комплексной физиотерапии и на основе этого — статистическая оценка предикторов удлинения периода активного долголетия.</p> <p><bold>Методы.</bold> Выполнено клиническое проспективное рандомизированное контролируемое сравнительное исследование с участием 80 пациентов в возрасте от 20 до 90 лет. Методом простой фиксированной рандомизации все пациенты были разделены на две группы: контрольную и основную. Пациенты контрольной группы получали умеренную физическую нагрузку, снижение калорийности питания и витаминотерапию. Пациенты основной группы дополнительно получали физиотерапевтический комплекс, в состав которого входили мультимодальные воздействия с использованием аппарата Alpha™ LED Оху Light Spa™, гипокси-гипероксическая терапия, прессотерапия, сухие углекислые ванны. Профилактические курсы повторялись дважды с интервалом в 6 месяцев. Все исследования проводились до каждого профилактического курса и через 6, 12 и 18 месяцев после его завершения. Для выявления предикторов были применены множественный линейный регрессионный анализ и корреляционная адаптометрия.</p> <p><bold>Результаты.</bold> Установлено, что комплексная физиотерапия снижает биологический возраст и скорость старения не только сразу после курса, но и в течение года. Эффект обусловлен уменьшением инсулинорезистентности, атерогенности и воспаления на фоне незначительного удлинения теломер. Предикторами долгосрочного результата являются низкие показатели инсулинорезистентности, воспаления и окислительного стресса при увеличенной длине теломер.</p> <p><bold>Заключение.</bold> Преждевременное старение обусловлено системными нарушениями, прежде всего в инсулиновой регуляции углеводного и липидного обмена. Применение методов восстановительной медицины (аппаратной физиотерапии и природных факторов) высокоэффективно, так как они безопасно стимулируют гормональную регуляцию метаболизма, доступны и просты в использовании.</p></trans-abstract><trans-abstract xml:lang="zh"><p>背景。<bold> </bold>由于研究的复杂性和患者的共病，活跃长寿计划的开发面临困难。多模式且安全的康复医学方法是一个有前景的解决方案。我们之前已证明综合物理治疗在降低生物年龄方面的有效性，但为了个性化预防，需要分析长期结果。</p> <p>目的。研究患者在应用综合物理治疗后的长期状态变化，并基于此对延长活跃长寿期的预测因素进行统计评估。</p> <p>方法。进行了一项临床、前瞻性、随机、对照、比较研究，涉及80名年龄在20至90岁之间的患者。通过简单固定随机化方法，所有患者被分为两组：对照组和干预组。对照组患者接受适度的体育锻炼、降低饮食热量和维生素治疗。干预组患者还接受了包括使用Alpha™ LED Oxy Light Spa™设备的多模式疗法、低氧-高氧治疗、压力疗法、干式二氧化碳浴在内的物理治疗综合疗法。预防性疗程每隔6个月重复两次。所有研究在每个预防性疗程之前以及完成后6、12和18个月进行。为了识别预测因素，应用了多元线性回归分析和相关适应度分析。</p> <p>结果。研究表明，综合物理治疗不仅在疗程结束后立即，而且在一年内降低了生物年龄和衰老速度。效果是由于伴随端粒轻微延长，减少了胰岛素抵抗、动脉粥样硬化和炎症。长期结果的预测因素是低胰岛素抵抗、炎症和氧化应激指标以及端粒长度增加。</p> <p>结论。过早衰老是由系统性紊乱引起的，主要是胰岛素对碳水化合物和脂质代谢的调节问题。采用恢复性医学的方法（设备理疗和自然因素）非常有效，因为它们能够安全地刺激代谢的激素调节，且易于使用。</p></trans-abstract><kwd-group xml:lang="en"><kwd>active longevity</kwd><kwd>comprehensive physiotherapy</kwd><kwd>long-term outcomes</kwd><kwd>predictors of effectiveness</kwd><kwd>insulin resistance index</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>активное долголетие</kwd><kwd>комплексная физиотерапия</kwd><kwd>отдалённые результаты</kwd><kwd>предикторы эффективности</kwd><kwd>индекс инсулинорезистентности</kwd></kwd-group><kwd-group xml:lang="zh"><kwd>积极长寿</kwd><kwd>综合理疗</kwd><kwd>远期结果</kwd><kwd>效果预测指标</kwd><kwd>胰岛素抵抗指数</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Korchazhkina NB, Mikhailova AA, Kotenko KV, et al.
