causes of mortality and disability worldwide, particularly in low- and middle-income countries. This review analyzes current approaches to cardiac rehabilitation in patients after acute coronary syndrome, with a focus on epidemiology, clinical features, and innovative strategies for cardiac rehabilitation programs. Key components of cardiac rehabilitation include physical activity, psychotherapeutic interventions, dietary therapy, and pharmacological treatment, all of which require a personalized approach to improve clinical outcomes. Special attention is given to gender differences in acute coronary syndrome progression and access to rehabilitation programs, as well as the impact of the COVID-19 pandemic on disease trends. Recent studies demonstrating the effectiveness of integrating digital technologies, telemedicine, and mobile applications into cardiac rehabilitation programs are reviewed. Both traditional rehabilitation methods and innovative approaches—including high-intensity interval training and the Mediterranean diet—are discussed, offering new perspectives for designing rehabilitation programs. Despite the proven effectiveness of cardiac rehabilitation, its implementation remains limited due to low patient adherence and insufficient accessibility in resource-limited regions. New clinical guidelines on optimizing rehabilitation strategies, aimed at reducing the risk of recurrent complications and improving patients’ quality of life, are currently under development.</p></abstract><trans-abstract xml:lang="ru"><p>Острый коронарный синдром (ОКС) остаётся одной из ведущих причин смертности и инвалидности во всём мире, особенно в странах с низким и средним уровнем дохода. Настоящий обзор посвящён анализу современных подходов к кардиореабилитации (КР) пациентов, перенёсших ОКС, с акцентом на эпидемиологию, клинические особенности и инновационные стратегии программ КР. Ключевые аспекты КР включают физическую активность, психотерапевтические методы, диетотерапию и медикаментозное лечение, требующие, в свою очередь, персонализированного подхода для улучшения клинических исходов. Особое внимание уделяется гендерным различиям в течении ОКС и доступе к реабилитационным программам, а также влиянию последствий пандемии COVID-19 на динамику заболевания. Проанализированы данные последних исследований, доказывающие эффективность интеграции цифровых технологий, телемедицины и мобильных приложений в программы КР. Авторами рассмотрены как традиционные методы реабилитации, так и инновационные подходы, включающие высокоинтенсивные интервальные тренировки и средиземноморскую диету, что позволяет рассматривать новые подходы к разработке реабилитационных программ. Несмотря на доказанную эффективность КР, её внедрение остаётся ограниченным из-за низкой комплаентности пациентов и недостаточной доступности в регионах с ограниченными ресурсами.
