A critical issue in pediatric surgery is the development of effective rehabilitation strategies following proctoplasty. Medical rehabilitation aims to normalize bowel transit, restore the tone of the rectal sphincter and pelvic floor muscles, reestablish the defecation reflex, and reduce asthenovegetative dysfunctions.</p> <p><bold>AIM:</bold> To substantiate the use of extracorporeal magnetic stimulation in rehabilitation of children who underwent surgery for anorectal malformations.</p> <p><bold>MATERIALS AND METHODS:</bold> As part of an open controlled clinical study, 77 children (mean age 5.99±3.22 years) who had undergone surgery for anorectal malformations were monitored over time. They were included into either of two groups: 39 children received extracorporeal magnetic stimulation, whereas 38 children did not have any physiotherapeutic interventions. All children participated in therapeutic exercises for strengthening the pelvic floor muscles. To evaluate treatment efficacy, ultrasound imaging of the perineum and pelvic floor structures was performed, along with Doppler ultrasonography of the rectal vasculature.</p> <p><bold>RESULTS:</bold> Extracorporeal magnetic stimulation was associated with a positive clinical response, including improved colonic evacuatory function and decreased episodes of fecal soiling. Ultrasound demonstrated statistically significant increases in the anal canal width, levator muscle dimensions (<italic>p</italic>=0.04), external sphincter thickness on the right (<italic>p</italic>=0.004) and left (<italic>p</italic>=0.012) sides, and puborectalis sling size (<italic>p</italic>=0.009). A trend toward improved intramural blood flow parameters was observed based on Doppler ultrasonography of the pull-through rectum. However, long-term follow-up at 3, 6, and 12 months indicated a gradual decline in the achieved therapeutic effects.</p> <p><bold>CONCLUSIONS:</bold> The studies showed that extracorporeal magnetic stimulation has a positive impact on key clinical and functional parameters in children with anorectal malformations. Analysis of long-term outcomes supports the feasibility of repeated extracorporeal magnetic stimulation courses to maintain therapeutic benefits.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Аноректальные мальформации занимают важное место в группе врождённых пороков развития у детей. Актуальной проблемой является разработка технологий медицинской реабилитации детей после проктопластики. Медицинская реабилитация направлена на нормализацию пассажа кала, тонуса сфинктерного аппарата прямой кишки и мышц тазового дна, восстановление рефлекса на дефекацию, уменьшение астеновегетативных нарушений.</p> <p><bold>Цель исследования</bold> — обосновать применение экстракорпоральной магнитной стимуляции у детей, оперированных по поводу аноректальных мальформаций.</p> <p><bold>Материалы и методы.</bold> В рамках открытого контролируемого клинического исследования обследовано 77 детей в возрасте 5,99±3,22 лет, оперированных по поводу аноректальных мальформаций. Детей разделили на две группы: 39 получали экстракорпоральную магнитную стимуляцию, 38 не получали методы физиотерапии. Все дети получали лечебную гимнастику, включающую упражнения для укрепления мышц тазового дна. С целью оценки эффективности проводимого лечения использовали методы ультразвукового исследования промежности и структур тазового дна с допплерографией сосудов прямой кишки.</p> <p><bold>Результаты. </bold>На фоне проведения экстракорпоральной магнитной стимуляции выявлена положительная динамика основных клинических симптомов в виде улучшения эвакуаторной функции толстой кишки и уменьшения эпизодов каломазания. По данным ультразвукового исследования регистрировали статистически значимое увеличение ширины анального канала, размеров мышечных леваторов (<italic>р</italic>=0,04), толщины наружного сфинктера справа (<italic>р</italic>=0,004) и слева (<italic>р</italic>=0,012), размеров пуборектальной петли (<italic>р</italic>=0,009). Отмечена тенденция к улучшению показателей интрамурального кровотока по данным допплерографии сосудов низведённой прямой кишки. Отдалённые результаты через 3, 6, 12 мес. свидетельствуют о снижении достигнутых положительных эффектов.</p> <p><bold>Заключение.</bold> На основании проведённых исследований установлено положительное действие экстракорпоральной магнитной стимуляции на основные клинико-функциональные показатели у детей с аноректальными мальформациями. Анализ отдалённых результатов обосновывает целесообразность проведения повторных курсов экстракорпоральной магнитной стимуляции.</p></trans-abstract><trans-abstract xml:lang="zh"><p>现实意义。肛门直肠畸形在儿童先天性发育畸形中占有重要地位。直肠成形术后儿童医疗康复技术的研发是一个亟待解决的问题。医疗康复是针对排便的正常化、增强直肠括约肌、骨盆底肌肉的张力、恢复排便反射和降低自主神经功能障碍。</p> <p>研究目的<bold> </bold>— 论证体外磁刺激在儿童肛门直肠畸形手术后的应用合理性。</p> <p>材料和方法。在一项开放式对照临床研究中，接受调查的是77名肛门直肠畸形手术的儿童，动态年龄为5.99±3.22岁，分为两组：39名儿童接受了体外磁刺激，38名儿童没有接受物理治疗。所有儿童都接受了运动疗法，包括加强骨盆底肌肉的锻炼。为了评估治疗效果，对会阴和盆底结构进行了超声检查，并对直肠血管进行了多普勒超声检查。</p> <p>结果。在体外磁刺激下，主要临床症状出现了积极的动态变化，表现为大肠排便功能得到改善，以及大便失禁发作次数的减少。根据超声波检查数据，肛门通道、肌肉外展肌大小（p=0.04）、右侧（p=0.004）和左侧（p=0.012）外括约肌厚度、耻骨直肠环大小（p=0.009）均有明显增加，且有统计学意义。根据直肠下段血管多普勒检查数据，腹腔内血流指标有改善的趋势。在3、6、12个月后，远期结果证实所取得的积极效果下降。</p> <p>结论。根据已进行的研究确定，体外磁刺激对儿童肛门直肠畸形的主要临床功能指标具有积极作用。远程结果的分析证实，进行体外磁刺激重复疗程的合理性。</p></trans-abstract><kwd-group xml:lang="en"><kwd>medical rehabilitation</kwd><kwd>extracorporeal magnetic stimulation</kwd><kwd>children</kwd><kwd>anorectal malformations</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>медицинская реабилитация</kwd><kwd>экстракорпоральная магнитная стимуляция</kwd><kwd>дети</kwd><kwd>аноректальные мальформации</kwd></kwd-group><kwd-group xml:lang="zh"><kwd>医疗康复</kwd><kwd>体外磁刺激</kwd><kwd>儿童</kwd><kwd>肛门直肠畸形</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Narbaev TT, Aliev MM, Yuldashev AYu, Turayeva ZhT.
