One of these methods is laser therapy. The research hypothesis suggests that using laser stimulation of active points, it is possible to restore the synchronicity of contractile activity of the contralateral segments of the LV myocardium.</italic></p> <p><bold><italic>Aims: </italic></bold><italic>To study the effect of laser correction on the functional state of the LV myocardium in children and adolescents and the vegetative status, which leads to the formation of functional changes in the heart in children and adolescents, necessary for the development of complex therapy for systolic dysfunction.</italic></p> <p><bold><italic>Material and methods.</italic></bold><italic> The study involved 60 children and adolescents aged 12–16 years. Patients were divided into two groups: group 1 (n = 30) – children aged 12–14 years; 15 girls and 15 boys. Group 2 (n = 30) — teenagers 14–16 years old. The sample also includes 15 girls and 15 boys. Evaluation of contractile activity disorders was performed with ECHO-KG in the parasternal position along the long axis. The thickness of the interventricular septum (LVM) and the posterior wall of the left ventricle (LVL) in systole and diastole was evaluated. Assessment of the vegetative status was performed by cardiointervalography in the study of daily ECG recording. Treatment was performed by laser correction according to the standard method: 10 daily sessions with laser stimulation of trigger zones in the shoulder and forearm – extraceptive (1000 Hz, 2 min) and proprioceptive (50 Hz, 2 min), as well as in the direct projection of the heart (5 Hz, 2 min). </italic><italic>А</italic><italic>ctivation </italic><italic>р</italic><italic>oints: 4th intercostal space at the left edge of the sternum (5 Hz, 5 min), carotid sinus (left carotid artery pulsation zone — 50 Hz, 2 min), to the left of the spine at the level of the left corner of the scapula (5 Hz, 2 min). After laser therapy, repeated ECHO-cardiography was performed to assess the dynamics of LV systolic function in children and adolescents.</italic></p> <p><bold><italic>Results.</italic></bold><italic> Of the 60 children examined, 24% of children aged 12 to 14 years had symmetrical thickening of the myocardium of the LV and LVL, compared to 67% in the group aged 15 to 16 years. In the group from 12 to 14 years, 43% of children had asynchronous systolic dysfunction of the anterior-septal type. The remaining children (33%) had asynchronous posterobasal systolic dysfunction. In the group of children aged 14 to 16 years, asynchronous systolic dysfunction in the anterior-septal type was detected in 23%, and in the posterior-basal type — in 1%. According to the results of cardiointervalography, hypersympathicotonia was detected in 72% of children aged 14–16 years. Mixed type of vegetative dystonia was observed in 25% of cases in this age group. Hyperparasympathicotonia was detected in 3% of the examined patients. In children aged 12–14 years, a mixed type of hyperactivity was detected in 79% of cases. Hyperparasympathicotonia was observed in 15% of the examined patients, against 6% of patients with hypersympathicotonia. All children identified moderate stress tension autonomic regulation, and baroreflexes regulation was not violated. After laser therapy using a standardized method, the following results were obtained: in the group of children aged 12–14 years, symmetrical thickening of the contralateral segments of the myocardium was detected in 72% of children. In children aged 15–16, resynchronization was achieved in 85% of patients.</italic></p> <p><bold><italic>Conclusions.</italic></bold><italic> Laser therapy in children and adolescents with functional disorders of the contractile activity of the LV myocardium in the group of children 12–14 years old led to resynchronization of contraction of contralateral segments and a change in autonomic reactivity mainly to parasympathicotonic.</italic></p></abstract><trans-abstract xml:lang="ru"><p><bold><italic>Обоснование</italic></bold><bold>. </bold><italic>В связи с необходимостью разработки комплексной терапии детей и подростков с функциональными нарушениями работы сердца и нарушениями ритма на фоне перестройки вегетативной регуляции сердечной деятельности очень важным разделом является изучение возможности использования у пациентов лазеротерапии с целью коррекции нарушений систолической функции миокарда левого желудочка (ЛЖ). Одним из таких методов является лазеротерапия. Исследовательская гипотеза предполагает, что при помощи лазерной стимуляции активных точек возможно восстановить синхронность сократительной активности контралатеральных сегментов миокарда ЛЖ.</italic></p> <p><bold><italic>Цель</italic></bold><italic> — изучение влияния лазерной коррекции на функциональное состояние миокарда ЛЖ у детей и подростков и вегетативный статус, приводящей к формированию функциональных изменений в работе сердца у детей и подростков, необходимых для разработки комплексной терапии систолической дисфункции.</italic></p> <p><bold><italic>Материалы и методы.</italic></bold><italic> В исследовании участвовали 60 детей и подростков в возрасте 12–16 лет. Пациентов разделили на две группы: 1-я группа (n = 30) — дети (15 девочек и 15 мальчиков) в возрасте 12–14 лет; 2-я группа (n = 30) — подростки (15 девочек и 15 мальчиков) 14–16 лет. Оценку нарушения сократительной активности проводили при эхокардиографии в парастернальной позиции по длинной оси. Оценивали толщину миокарда межжелудочковой перегородки и задней стенки левого желудочка в систолу и диастолу. Оценку вегетативного статуса проводили методом кардиоинтервалографии при исследовании записи суточной электрокардиограммы. Проведено лечение методом лазерной коррекции по стандартной методике: 10 ежедневных процедур со стимуляцией лазером триггерных зон в области плеча и предплечья (для экстрацептивных — 1000 Гц, 2 мин; для проприоцептивных — 50 Гц, 2 мин), а также в прямой проекции сердца (5 Гц, 2 мин). Точки активации: 4-е межреберье у левого края грудины (5 Гц, 5 мин), каротидный синус (зона пульсации левой сонной артерии — 50 Гц, 2 мин), слева от позвоночника на уровне левого угла лопатки (5 Гц, 2 мин). После лазерной терапии проведена повторная эхокардиография с целью оценки динамики показателей систолической функции миокарда ЛЖ у детей и подростков.</italic></p> <p><bold><italic>Результаты.</italic></bold><italic> Симметричное утолщение миокарда межжелудочковой перегородки и задней стенки левого желудочка выявлено у 24% детей в возрасте от 12 до 14 лет и у 67% в группе 15–16-летних. В группе детей от 12 до 14 лет асинхронная систолическая дисфункция по переднеперегородочному типу выявлена в 43% случаев, асинхронная систолическая дисфункция по заднебазальному типу — в 33%; среди 14–16-летних — в 23 и 1% случаев соответственно. По результатам кардиоинтервалографии гиперсимпатикотония выявлена у 72% детей 14–16 лет и 6% пациентов 12–14 лет; смешанный тип вегетативной дистонии — у 25 и 79%, гиперпарасимпатикотония — у 3 и 15% обследованных соответственно. У всех детей выявлено умеренное стрессорное напряжение вегетативной регуляции без нарушений барорефлекторной регуляции. Лазеротерапия по стандартизированной методике в группе детей 12–14 лет продемонстрировала симметричное утолщение контралатеральных сегментов миокарда в 72% случаев, среди 15–16-летних — в 85%.</italic></p> <p><bold><italic>Заключение.</italic></bold><italic> Лазеротерапия у детей и подростков с функциональными нарушениями сократительной активности миокарда ЛЖ в группе детей 12–14 лет привела к ресинхронизации сокращения контралатеральных сегментов и изменению вегетативной реактивности преимущественно на парасимпатикотоническую.</italic></p></trans-abstract><kwd-group xml:lang="en"><kwd>vegetative regulation</kwd><kwd>systolic dysfunction</kwd><kwd>asynchronous myocardial contraction</kwd><kwd>resynchronization</kwd><kwd>laser therapy</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>вегетативная регуляция</kwd><kwd>систолическая дисфункция</kwd><kwd>асинхронное сокращение миокарда</kwd><kwd>ресинхронизация</kwd><kwd>лазеротерапия</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Malyshenko NM, Popova NS.
