Laser ablation of the fistula tract — the classic FiLaC technology was performed in 90 patients; modified FiLaC technology — 90. Each of the subgroups was divided into 3 groups: group 1 (main) included 30 patients who underwent a 4-component rehabilitation complex from the second day after the operation; group 2 (comparison) — 30 patients who underwent a 2-component rehabilitation complex from the second day after the operation and group 3 (control) — 30 patients, without an additional physiotherapeutic rehabilitation program. A comprehensive assessment of the features of the formation of postoperative scars was carried out as a criterion for the effectiveness of treatment, one of the tools of which was a complex transrectal ultrasound with color Doppler mapping, Dopplerography and sonoelastography at different stages of treatment.</p> <p><bold><italic>RESULTS:</italic></bold> All patients tolerated the course of treatment well: there were no cases of interruption of rehabilitation programs. The dynamic analysis of the data showed a statistically significant result: the best — in patients of the main group against the background of a 4-component rehabilitation program, the formation of postoperative scars accelerated, the formation of a mature scar occurred earlier, the risks of developing postoperative complications and relapses of the disease decreased.</p> <p><bold><italic>CONCLUSION:</italic></bold> The use of rehabilitation programs in patients with chronic paraproctitis in the early postoperative period after minimally invasive surgical hospital-replacing technologies can improve clinical results due to the complex physiotherapeutic effect. Complex transrectal ultrasound with color Doppler mapping, Dopplerography and sonoelastography in patients with chronic paraproctitis is an effective non-invasive diagnostic method: monitoring the formation of a postoperative scar and assessing the ongoing treatment.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Изучение особенностей формирования послеоперационных рубцов под влиянием реабилитационных программ у больных хроническим парапроктитом в раннем послеоперационном периоде после малоинвазивных хирургических стационарозамещающих технологий расширяет наше представление о данном патологическом состоянии и способах его лечения.</p> <p><bold>Цель исследования</bold> <bold>—</bold> изучить влияние различных реабилитационных программ на формирование послеоперационных рубцов у больных хроническим парапроктитом в раннем послеоперационном периоде после малоинвазивных хирургических стационарозамещающих технологий.</p> <p><bold>Материал и методы.</bold> В исследовании обобщён и проанализирован опыт хирургического лечения 180 больных с хроническим парапроктитом с использованием малоинвазивных хирургических стационарозамещающих технологий и реабилитационных программ в раннем послеоперационном периоде, разделённых на две группы по 90 человек. Лазерная абляция свищевого хода — классическая технология FiLaC — выполнена у 90 пациентов; модифицированная технология FiLaC — также у 90 человек. Каждая из групп была разделена на 3 подгруппы: в подгруппу 1 (основную) было включено 30 пациентов, которым начиная со второго дня после операции проводился четырёхкомпонентный реабилитационный комплекс; подгруппу 2 (сравнения) — 30 пациентов, которым начиная со второго дня после операции проводился двухкомпонентный реабилитационный комплекс; подгруппу 3 (контрольную) — 30 пациентов без дополнительной физиотерапевтической реабилитационной программы.</p> <p>Проводилась комплексная оценка особенностей формирования послеоперационных рубцов как критерия эффективности лечения, одним из инструментов которого являлось комплексное трансректальное ультразвуковое исследование с цветовым допплеровским картированием, допплерографией и соноэластографией на разных этапах лечения.</p> <p><bold>Результаты. </bold>Все пациенты хорошо перенесли курс лечения: случаев прерывания реабилитационных программ не было. Проведённый динамический анализ данных показал статистически значимый результат: лучший — у пациентов основной подгруппы на фоне проведения четырёхкомпонентной программы реабилитации, у которых ускорились процессы формирования послеоперационных рубцов, наступало более раннее формирование зрелого рубца, уменьшились риски развития послеоперационных осложнений и рецидивов заболевания.</p> <p><bold>Заключение. </bold>Использование реабилитационных программ у больных хроническим парапроктитом в раннем послеоперационном периоде после малоинвазивных хирургических стационарозамещающих технологий позволяет улучшить клинические результаты за счёт комплексного физиотерапевтического воздействия.
