The attention of many researchers is drawn to L-carnitine and its role in the metabolism of fats and carbohydrates in both normal and pathological conditions.</p> <p>The review is devoted to an urgent topic — the theoretical justification of increasing the biogenic potential of drinking mineral waters in order to use them in the non-drug treatment of metabolic syndrome. The choice of a biologically active substance for the modification of drinking mineral waters, namely, L-carnitine, is based on its direct involvement in the energy exchange of body cells.</p> <p>L-carnitine plays an irreplaceable role in lipid metabolism: it participates in the transport of fatty acids and products of their incomplete oxidation across the mitochondrial membrane, potentiating β-oxidation of long-chain fatty acids, which leads to the activation of energy metabolism in various tissues. Thus, it can be assumed that the addition of L-carnitine to the diet of patients with metabolic syndrome and type 2 diabetes mellitus may be an effective tool in the treatment and prevention of the progression of these diseases.</p> <p>The data of experimental and clinical studies justifying the preventive effect of drinking mineral waters were analyzed. In addition, we have conducted searching for scientific publications over the past 5 years in the electronic databases PubMed, Web of Science, e-Library on the study of L-carnitine and drinking mineral waters of various compositions, taking into account their influence on the components of metabolic syndrome. The search terms provided criteria that included full text, related data, clinical trials, meta-analysis, randomized controlled trials, and systematic reviews.</p> <p>Based on the results of independent scientific studies, it is unambiguous to conclude that there is both a direct (due to dissolved ions) and a more complex (mediated through regulatory systems) effect of mineral waters on impaired metabolism caused by magnesium and calcium deficiency in this group of patients.</p></abstract><trans-abstract xml:lang="ru"><p>В настоящее время проводится поиск немедикаментозных биологически активных средств, способствующих восстановлению нарушенного метаболизма при метаболическом синдроме. Внимание многих исследователей обращено к L-карнитину и его роли в обмене жиров и углеводов как в норме, так и при патологии.</p> <p>Обзор посвящён актуальной теме — теоретическому обоснованию повышения биогенного потенциала питьевых минеральных вод с целью использования их в немедикаментозном лечении метаболического синдрома. Выбор биологически активного вещества для модификации питьевых минеральных вод, а именно L-карнитина, основан на его непосредственном участии в энергообмене клеток организма.</p> <p>L-карнитин играет незаменимую роль в липидном обмене: участвует в транспорте жирных кислот и продуктов их неполного окисления через митохондриальную мембрану, потенцируя β-окисление длинноцепочечных жирных кислот, что приводит к активации энергообмена в различных тканях. Таким образом, можно предположить, что добавление L-карнитина в рацион пациентов с метаболическим синдромом и сахарным диабетом 2-го типа может оказаться эффективным инструментом в лечении и профилактике прогрессирования этих заболеваний.</p> <p>Проанализированы данные экспериментальных и клинических исследований с обоснованием превентивного действия питьевых минеральных вод. Кроме того, проведён поиск научных публикаций за последние 5 лет в электронных базах данных PubMed, Web of Science, eLibrary по исследованию L-карнитина и питьевых минеральных вод различного состава с учётом их влияния на компоненты метаболического синдрома. В условиях поиска были предусмотрены критерии, которые включали полный текст, связанные данные, клинические испытания, метаанализ, рандомизированные контролируемые испытания и систематические обзоры.</p> <p>По результатам независимых научных исследований, однозначным является вывод о наличии как прямого (вследствие растворённых ионов), так и более сложного (опосредованного через системы регуляции) влияния минеральных вод на нарушенный метаболизм, обусловленный дефицитом магния и кальция, у данного контингента пациентов.</p></trans-abstract><kwd-group xml:lang="en"><kwd>L-carnitine</kwd><kwd>drinking mineral water</kwd><kwd>metabolic syndrome</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>L-карнитин</kwd><kwd>питьевые минеральные воды</kwd><kwd>метаболический синдром</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Oganov RG, Bakulin IG, Simanenkov VI, et al.
